APPENDIX A NOTICE OF VIOLATION f
Philadelphia Electric Company Docket No.: 50-278 Peach Bottom Unit 3 License No.: DPR-56 As a result of the inspection conducted on March 22 - May 9, 1986, and in accordance with revised NRC Enforcement Policy (10 CFR 2 Appendix C),
published in the Federal Register on March 8, 1984 (49 FR 8583) the following violation was identified:
Technical Specification surveillance requirement 4.6.E states that whenever there is recirculation flow with the reactor in the run mode, jet pump operability shall be checked daily by verifying that the following conditions 4
do not occur simultaneously:
(a) The indicated value of core flow rate varies from the value derived from loop flow measurements by more than 10's.
)
(b) The diffuser to lower plenum dif ferential pressure reading on an individual jet pump varias from the mean of all jet pump differential pressures by more than 10*s.
l Additionally, the diffuser to lower plenum differential pressure shall be j
checked daily and the differenti,al pressure of any jet pump in the idle loop 4
shall not vary by more than 10*s from established pattern.
l Contrary to the above, from 6:11 a.m. on March 27, 1986, to 4:30 p.m.,
on j
March 31, 1986, the licensee operated in single loop on Unit 3 without verifying that the conditions (a) and (b) identified above did not occur i
simultaneously nor were they checking the status of the idle loop as i
required.
