This refers to the routine safety inspection conducted by Mr. S. M. Hare of-

this office on January 19 through March 13, 1986, of activities at Palisades
!
Nuclear Plant authorized by NRC Operating License No. DPR-20 and to the
j
discussion of our findings with Mr. J. firlit and members of his staff at the
conclusion of the inspection.

The enclosed copy of our inspection report identifies areas examined during.

the inspection. Within these areas, the inspection consisted of a selective
i
examination of procedures and representative records, observations, and-
interviews with personnel.

During this inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Notice of Violation. A
written response is required.
i
As stated in the report, th'e last two consecutive as-found Type A tests at
~
?.
Palisades have been unsuccessful and, pursuant to 10 CFR 50, Appendix J,.
l
Section III.A.6.(b), a Type A test shall be perfonned during each plant
shutdown for refueling until two consecutive Type A tests meet the acceptance
]
criteria of Appendix J.
Accordingly, unless you request and are granted an
j
exemption from this requirement'by the Office of Nuclear Reactor Regulation, we
expect you to conduct a Type A test during each refueling outage untilL two

consecutive Type A tests satisfy the Appendix J acceptance criteria.
