Gentlemen:

SUBJECT: REPORT NOS. 50-259/86-10, 50-260/86-10, AND 50-296/86-10

On March 10-12, 1986, NRC inspected activities authorized by NRC Operating

License Nos. DPR-33, DPR-62, and DPR-68 for your Browns Ferry facility. At the

conclusion of the inspection, the findings were discussed with those members of

your staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of activities

in progress.

The inspection findings indicate that certain activities violated NRC require-

ments. The violations references to pertinent requirements, and elements to be

included in your respon,se are presented in the enclosed Notice of Violation.

In the exit interview, plant management requested that the issue regarding

entries into radiation work permit areas and contamination zones by personnel

without current training and whole body counts be considered licensee identified.

We have reviewed this request in light of the self-identification criteria in

10 CFR 2, Appendix C, Section V. A and determined that the issue did not meet all

the criteria listed in 10 CFR 2 in that the issue was not corrected, including

measures to prevent recurrence, within a reasonable time.

In addition, during the exit interview, plant management requested that the issue

involving the uptake of radioactive material by a worker on September 27, 1985,

be considered self-identified. We.have reviewed the circumstances involving this

clearance procedures of the Office of Management and Budget issued under the

Paperwork Reduction Act of 1980, PL 96-511.
