Inspection No. 50-333/88-15
This letter refers to the special operational safety inspection conducted by a
team of NRC personnel led by Dr. P. K. Eapen of this office on August 8, 1988,
to August 19, 1988, at the James A. FitzPatrick Nuclear Power Plant and to the
discussion of our findings held by Dr. Eapen with you and members of your staff
at the cor.:1usion of the inspection.
Areas examined during this insaection included systems and plant personnel
actions required to mitigate tie consequences of accident sequences which have
the potential to result in core melt.
The systems and accident sequences were
selected from generic probabilistic risk studies for Boiling Water reactors and
were modified to reflect specific features at the James A. FitzPatrick Nuclear
Power Plant.
The systems selected were the on-site and off-site electrical
power systems, Emergency Core Cooling Systems (HPCI, RCIC, and RHR), the
Emergency Diesel Generators, Emergency Service Water Systems, Instrument Air
System, Main Steam Isolation Yalves and the Automatic Depressurization System.
For these systens the inspection focused on the availability of critical
components and support systems.
The inspection also reviewed system operating
experience, surveillance tests, and maintenance activities to determine component
availability.
In addition, this inspection assessed the ability of the licensee
staff to use existing procedures and )lant equipment to respond to and recover
from certain postulated accidents. T1e assessments were made by conducting
procedure walk throughs with licensee personnel in the facility and by observing
the performance of qualified personnel during selected scenarios on a plant
specific control board mockup. The findings of this inspection are described
in the enclosed NRC Region I Inspection Report.
Based on the results of this inspection, it appears that certain of your
activities were not conducted in full compliance with the NRC requirements, as
set forth in the Notice of Violation and the Notice of Deviation, enclosed
herewith as Appendix A and Appendix B respectively. The violation has been
categorized by Severity Level in accordance with the General Statement of
Policy and Procedures for NRC Enforcement Actions, 10 CFR Part 2, Appendix C.
You are required to respond to this letter and in preparing your response, you
should follow the instructions in Appendix A and Appendix S.

Overall, the results of this inspection indicate that the plant equipment and
licensee staff are capable of reliable response to the accidents postulated
during this inspection.
In addition, the blackout procedure and PRA Study,
currently under development by your staff, will enhance your accident response
capabilities further. This conclusion assumes that the existing management
initiatives and maintenance efforts to reduce equipment down time will continue
in the future. The inspection report suninarizes other positive attributes such
as tenior management initiatives to maintain high availability for plant equip-
ment, and capable operations and maintenance staff.
Several negative observations, such as, lack of engineering support for requested
plant modifications, lack of spare oil filters for the HPCI System and inadequate
procedure; for the blowdown of diesel generator air receivers, were identified
and resolved during the inspection.
Our inspectors also identified a weakness
in the control of your battery room environment as detailed in section 2.3 of
the enclosed report.
In particular, we are concerned that the numerous inoperable
components, the unavailability of replacement parts and the lack of monitoring
and procedural guidance for the Low Pressure Coolant Injection tattery room
ventilation system may adversely impact the availability of thr. batteries.
Therefore, you are requested to review this concern and take required actions
to maintain the battery room environment in a reliable manc.er.
Additionally,
one unresolved item (50-333/88-15-03), as detailed in s% tion 4.2.B of the
enclosed report and related to the root cause for the High Pressure Coolant
Injection turbine auxiliary lube oil pump pressure switch coming loose during
operation, was identified.
You are requested to respond to the above weakness
and unresolved item in your response to this letter.
The responses requested by this letter and the accompanying notice are not
subject to the clearance procedures of the office of management and budget as
required by the Paper Work Reduction Act of 1980, PL 96-511.
