REVIEW During the NRC/TVA Restart and Stello Plan Key Issues Schedular Review Meeting of February 5-6, 1987, a presentation was made on the Design Baseline and Verification Program (DBVP) review of PMTs performed at SQN. After the presentation, NRC staff members requested that a submittal be prepared by TVA to address the disposition of the PMTs determined to be inadequate by the PMT review. The requested information is provided in the enclosure.

I would like to point out that we are in the process of establishing a more formal restart test program at Sequoyah to better address all required restart testing. We are patterning our program to incorporate the better features of the Davis-Besse program. We will be willing to discuss additional details af ter the program development is complete (currently scheduled for mid-May).

We have also obtained the services of key members of the Davis-Besse team to help us implement this program in a conscientious, coherent fashion.

REQUESTED INFORNATION ON DBVP PNT REVIEW NRC/TVA RESTART AND STELLO PLAN KEY ISSUES SCHEDULAR REVIEW NEETING ON FEBRUARY 5-6, 1987 As requested in the subject meeting TVA has compiled the items from the DBVP PNT review which require additional data to satisfy-PNT. verification. The attached data represents the items to date for which the determination has I

been made that the Engineering Change Notice (ECN), Field Change Notice (FCN),

i or Workplan (WP) documented testing is inadequate. The data is presented in two groups: Table 1 contains the items for which other documented testing or I

operational activities have satisfactorily verified the functional operability for restart of the affected component, subsystem, or system, since the modification; Table 2 contains the items which were identified by the Division of Nuclear Engineering (DNE) as requiring retest.

By unit 2 restart, DNE will have reviewed all unit 2 and acommon ECN/FCN modification packages which were incomplete at, or initiated since, issuance of the unit 2 operating license and are associated.with the 37 systems or j

portions of systems necessary to mitigate the Final Safety Analysis Report 1

(FSAR) Chapter 15 postulated events.

As part of this review, all of the l

above-mentioned ECN/FCN packages which are to be implemented or partially implemented at unit 2 restart will have been reviewed to determine if the t

4 modifications were or will be adequately tested'to verify the functional operability for restart of the modified components, subsystems, and systems.

I implementing WP documentation to fully assess the scope of the implemented or proposed modification. The implementation documentation is then reviewed'to 1

evaluate the adequacy of the documented PNT.

If this review yields PNT l-inadequacies, the review is expanded to other plant programs in an attempt to locate sufficient documentation to satisfy the need for verification of i

functional operability for restart.

If the documentation review fails to disclose the existence of documented evidence which adequately confirms functional operability for restart after the modification activities, additional testing is specified and added to the

punchlist as a restart prerequisite. These review results are also documented in the appropriate DBVP System Evaluation Reports (SYSTERs).

. In summary, the program described above causes every implemented or partially implemented (at restart) ECN and its associated WPs to be reviewed by each technical discipline (civil, electrical, mechanical, and nuclear), such that all applicable functional testing aspects are considered.

PMT review guidelines are used by the disciplines in their respective evaluation.

As part of the DBVP efforts, the DBVP System Engineer (SE) has the overall responsibility for the equipment in his assigned system.

In this capacity, the SE has the responsibility to ensure PMT is adequately addressed or identified as a restart prerequisite in the punchlist.

The SE's evaluations and conclusions are reviewed by an independent, equally qualified individual as part of the ECN checklist (SQEP-12) development and review process.

A further review is conducted by an independent reviewer, a Senior Level Engineer (SLE), and the DNE review board as part of the SYSTER review and approval process as described in SQEP-16, " Procedures for System Evaluation and Development of System Evaluation Report," and SQEP-31, "DNS Review Board."

By restart, the DBVP program will have reviewed all implemented and partially implemented (at restart) design changes completed between the issuance of the operating license and the implementation of SQEP-13. " Procedure for Transitional Design Control Process." The postmodification testing aspect of each of the changes is evaluated as an integral part of the DBVP process.

This process is adequately comprehensive to reveal all postmodification testing deficiencies in design changes implemented since issuance of the operating license.

The overall safety significance of the review findings to date is minimal.

For the items listed in table 1, there is no significant operability concern since a later program test has demonstrated functional operability for restart. For the completed items listed in table 2, safety reviews were performed by the Plant Operations Review Staff (PORS) of the deficiencies identified by DNE.

To date, one Potentially Reportable Occurrence (PRO) report was generated as a result of the PORS review, and it was determined to be not reportable (PRO-1-87-041).

Another review is performed af ter the retest is completed to evaluate any test deficiencies for safety significance.

To date, no retest deficiencies have been identified which would indicate that the modification was improperly or incorrectly implemented. Two of the items yet to be retested have Significant Condition Reports (SCRs) generated on them.

SQN EEB 8515 was determined to not be reportable, and SQN NEB 8708 has not yet been issued by DNE.
