Dear Mr. Keppler:

This letter is in response to Mr. W. G. Guldemond's letter dated July 2, 1986, which forwarded the report on the routine safety inspection conducted by members of your staff. This inspection was conducted from May 13 through June 16, 1986 on activities at the D. C. Coold Nuclear Plant Units 1 and 2.

The Notice of Violation attached to Mr. Guldemonds's letter identified one violation. This violation is addressed in the attachment to this letter.

This document.has been prepared following corporate procedures which incorporate a reasonable set of controls to insure its accuracy and completeness prior to signature by the undersigned.

NRC Violation

" Unit 1. Technical Specification 6.8.1.a requires written procedures be established, implemented and maintained covering the applicable procedures recommended in Regulatory Guide 1.33 Appendix A, November 1972, which includes Administrative Procedures for equipment control and for procedure review and approval.

Plant-Manager's Instruction PMI-2010, ' Plant Manager and Department Head Instructions, Procedures, and Associated Indexes,' an administrative procedure for equipment control and procedure review and approval requires in Paragraph 3.7.1 that procedures be prepared utilizing the format specified in the associated Attachments 1 and 2., ' Format and Procedures' at Paragraph 4.I entitled, ' Restoration' requires steps be provided to assure necessary restoration of all controls and equipment whose status was changed during the procedure.

Contrary to the above, licensee Proceduee **1 THP 6030 IMP.076, 'Incore Thermocouple and Reactor Coolant System RTD Cross Calibration Procedure' provides for installation of calibrated pressure gauges at various points in the main steam system, but does not contain steps 'to remove the installed gauges on completion of the test.

As a consequence, following completion of the subject test on October 31, 1985 with the plant in Mode 3, four pressyre gauges, one upstream of each of the four main steam isolation valves, were not removed.

The plant subsequently operated in Mode 1 for an extended period with the gauges still installed."

Corrective Action Taken and Results Achieved The four pressure gauges were removed from the Unit 1 steam generat'or steam pressure sensing lines prior to June 5, 1986.

Corrective Action Taken To Avoid Further Violations Procedure 12 MHP 4030.STP.002 " Maintenance Procedure for Main Steam System-Safety Valve Set Point Verification (Secondary System Safety Valve Settings)" was revised on July 16, 1986.

Procedure 2 THP 6030. IMP.276 was revised by June 15, 1986, and 1 THP 6030. IMP.076 will be revised by August 15, 1986.

These revisions will insure that removal of the main steam system teet gauges occurs at the conclusion of the testing portions of the procedures.

In addition, the Maintenance Department has completed a review of their procedures in which temporary instrumentation is used and found no further evidence of missed restoration steps. The C&I/ Planning Department will select a random sample of their surveillance procedures for review to confirm that the absence of the restoration steps noted above was an isolated incident.

The sampling will be based on specific quality assurance guidelines, and the the number of procedures in the sample will be incrAased if warranted by the review.

The review will be completed by December 1, 1986.

If changes to any procedure are required, they will be completed prior to the next use of the procedure, but no later than its next biennial review.
