SECOND INTERIM REPORT Description of Deficiency During a design review of design change requests (DCRs), an analysis section responsible for rigorous analysis of Bellefonte Nuclear Plant (BLN) piping identified three DCRs which were not routed to the section even though the design changes may have affected the rigorous analysis. Per the BLN Engineering Project (BLEP) Engineering Procedure (EP) 44.76, the DCR is defineg as the document used by the piping designer to notify the piping analyst of a change to the piping design that has the potential of affecting the results of an issued analysis or to notify the analyst of the need to perform a new analysis. In the case of DCRs 1349 and 869, piping modifications with such a potential were not sent to the analysis section for review and in the case of DCR 2760, information in the DCR which was used by the anlaysis section does not agree with the design change information on the revised physical drawings issued by design personnel.
The cause of this condition was failure on the part of design personnel to follow BLP-EP 44.76 and provide DCRs to analysts for revision made to controlled design documents.
Interim Progress TVA is reviewing the analysis problems affected by the three identified DCRs and is revising the analysis according to the current design documents.
Also, a review of all analysis problems currently being reanalyzed or scheduled to be reanalyzed in the next year is being initiated and will be used as a sampling to determine the scope of this problem and whether additional review or reanalysis will be required.
To prevent a recurrence of this problem, the BLN Rigorous Analysis Handbook, section BLN-RAH-102, and -602 has been revised to require that all DCRs involving rigorous analysis be incorporated into the relevant controlled design documents before the rigorous analysis work is finalized. This will allow analysis personnel the opportunity to compare the reanalysis problem with the actual changes made in design before the problem is issued and the respective engineering change notice (ECN) is closed.
