This refers to the routine safety inspection conducted by Messrs. J. Johnson,
M. McBride and others of this office on February 1, 1985 to March 4, 1985 at the
Pilgrim Nuclear Power Station, Plymouth, Massachusetts, of activities authorized
by NRC License No. DPR-35 and to the discussions of our findings held by Mr.
J. Johnson with Mr. C. Mathis and other member:; of your staff periodically during
the inspection and at the conclusion of the inspection.
Areas examined during this inspection are described in the NRC Region I Inspection
Report which is enclosed with this letter. Within these areas, the inspection con-
sisted of selective examinations of procedures and representative records, inter-
views with personnel, and observations by the inspector.
Based on the results of this inspection, it appears that certain of your activities
were not conducted in full compliance with NRC requirements, as set forth in the
Notice of Violation, enclosed herewith as Appendix A.
These violations have been
categorized by severity level in accordance with the revised NRC Enforcement Policy
(10 CFR 2, Appendix C) published in the Federal Register Notice (49 FR 8583) dated
March 8, 1984.
You are required to respond to this letter and in preparing your
response, you should follow the instructions in Appendix A.
We are concerned that Violations A and B indicate that potentially serious
program weaknesses are present in your surveillance program. These weaknesses
include the failure te properly schedule and track a number of surveillance
tests during the startup sequence in December 1984 and January 1985, failure to
take timely correctivt- actions to surveillance testing problems indicated
during a QA audit, and failure to conduct a thorough followup investigation on
the missed surveillances. We have cited item 4 in Violation A and item 3 in
Violation B despite the fact that you identified these items, because your
corrective actions were not timely in the first instance and your identifica-
tion of the scope of the problem was inadequate in the second instance.
In
your response to the attached violations, please address these weaknesses.

We are also concerned with your recently implemented program for independently
verifying system configurations following maintenance and testing in response to
'TMI Action Plan Item I.C.6.
The' enclosed ~ inspection report identifies specific
programmatic concerns related;to 1)'the scope and systems to which the verification
program applies, 2) the' manner in which the verifications are being performed, and
3) the verification of maintenance work after which no surveillance testing is
performed. '-Th'ese c'oncerns are documented in three unresolved items in Section 8
of the enclosed. report. Along with your response to the violations described
~
above, please include your. plans to resolve these additional items.
