Description of the Problem The problem pertains to liquid penetrant examination of certain welds performed by ITT Grinnell.

During the - Nuclear Regulatory Commission Construction Appraisal Team Inspection, it was observed that some welds in the main. steam. safety and relief valve, vents, and drains system piping had

- inadequate surface preparation _ for the liquid penetrant examination.

Reexamination had also revealed some rejectable linear indications.

The reexamination by ITT Grinnell of previously examined accessible welds in the main steam and. relief valve, vents, and drains system has found that of i

- 188 welds inspected,151 were found to have discrepancies and or unacceptable surface conditions.

The 151 welds have since been reworked or repaired as required to an acceptable condition.

This reexamination is documented by Stone and Webster Type C Inspection Report No. QP4G0006.

In addition to the above, a reexamination was performed on other previously t

examined accessible welds to provide additional assurance of the ITT Grinnell liquid penetrant examination program.. The results of this examination were stated in our letter dated. May 4,1984 to Violation B, Example 5 of the Construction Appraisal Team Inspection..

Analysis of Safety Implications A detailed investigation of the welds that were repaired or rewelded was not l

performed to determine the effect of inadequate surface preparation - on the safety of operations of the plant because the corrective action detailed above has been taken.

However, since these welds are sa fety-related, a remote possibility existed that had this condition remained uncorrected, the safety of operations of the plant could have been adversely affected.

Corrective action has been taken as indicated above.

Other corrective and preventive actions were outlined in our letter dated May 4, 1984, to the Nuclear Regulatory Commission in response to the Construction Appraisal Team Inspection.
