Inspection Report No. 50-213/86-17

This refers to the special team inspection, conducted by Mr. A. G. Krasopoulos

of this office on June 16-20, 1986, at Haddam Neck Atomic Power Station of the

safe shutdown capability of the plant in the event of a fire and other activities

authorized by NRC License No. OPR-61 and to the discussions of our findings held

by Mr. Krasopoulos with Mr. E. J. Mroczka of your staff at the conclusion of the

inspection.

Areas examined during this inspection are described in the NRC Region I Inspec-

tion Report which is enclosed with this letter. Within these areas, the inspec-

tion consisted of selective examinations of procedures and representatives records,

interviews with personnel, and observations by the team.

The inspection determined that you do not currently satisfy all of the applicable

requirements of 10 CFR 50 Appendix R.

Exemption requests were previously granted

by the NRC for seven plant areas and a schedular exemption relating to the switch-

gear room and six other areas is currently under review by NRR.

For those areas

where compliance with Appendix R is currently required, four (4) potential vio-

lations were identified. These include:

the control room Halon system test failed

to meet the design basis; failure to demonstrate that cooldown could be accom-

plished using the steam generator vents if the atmospheric dump valves are

unavailable; inadequate breaker coordination setting procedures and inadequate

emergency lighting. One potential deviation was identified relating to a previous

commitment to lock open breakers for certain Reactor Coolant System Loop Bypass

Valves. Three items remained unresolved at the end of the inspection.

of these deficiencies are discussed in the enclosed inspection report.

The deviation is set forth in the Notice of Deviation enclosed herewith as

Appendix A.

You are requested to respond to the enclosed Notice, and in preparing

your response, you should follow the instructions specified therein.

The NRC plans to hold a meeting in the near future with your staff in Region I to

discuss the results of this inspection, the potential violations and the devia-

tion. NRC enforcement actions relating to the potential violations will be

deferred pending the outcome of this meeting.

Your corrective actions regarding the identified deficiencies should not be

delayed pending either a future NRC inspection or further action by the NRC

Region I office.

J. F. Opeka, CYAPC0 to C. I. Grimes, NRR dated July 3,1986.

If your planned

action or schedule for completing these actions changes, please notify me or

Dr. W. Johnston, Deputy Director, Division of Reactor Safety, RI or Mr. C. Grimes,

NRR, as appropriate.

The responses directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget, otherwise

required by the Paperwork Reduction Act of 1980, PL 96-511.

Information contained within the enclosed inspection report has been discussed'

with Mr. E. J. Mroczka of your staff on June 20, 1986. During this discussion

it was concluded that the inspection report contained no information that you

considered proprietary; therefore a copy of this letter and its enclosures are

being placed in the NRC's Public Document Room.
