Inspection Report No. 50-271/86-15
This refers to the routine resident safety inspection conducted by Mr. W. J. Raymond
of this office on July 1 - August 4, 1986, at the Vermont Yankee Nuclear Power
Station, Vernon, Vermont.
The inspection consisted of observation of activities,
interviews with personnel and document reviews.
The results of the inspection are
described in the NRC Region I Inspection Report enclosed with this letter and were
discussed with Mr. J. P. Pelletier of your staff at the conclusion of the inspec-
tion.
The inspector also reviewed the actions taken to correct the violation brought to
your attention in our letter dated January 6,1986, concerning the failure of tech-
nicians to follow and/or change approved procedures when the calibration radiation
sources used for a safety related monitor became weak.
We have no further ques-
tions regarding your corrective actions.
Based on the results of this inspection, it appears that one of your activities
was not conducted in full compliance with NRC requirements, as set forth in the
'
Notice of Violation enclosed herewith as Appendix A.
The violation concerns the
failure to determine the standby liquid control system liquid poison tank boron
concentration following water additions to the tank on two occasions. This viola-
tion has been categorized by severity level in accordance with the " General State-
ment of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix
C (Enforcement Policy 1986).
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your response,
you should document the specific actions taken and any additional actions you plan
,
to prevent recurrence.
The responses directed by this letter and the enclosed Notice are not subject to
the clearance procedures of the Office of Management and Budget as required by the
Paperwork Reduction Act of 1980, PL 96-511.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice", a copy of this
letter and the enclosures will be placed in the NRC Public Document Room.
