, Subject: Inspection Report No. 50-334/86-05

A routine safety inspection of your radiological controls and transportation

programs was conducted by Mr. P. Clemons of this office on March 10-14, 1986.

Areas examined during this inspection are described in the NRC Region I J

Inspection Report which is enclosed with this letter. Within these creas, the j

inspection consisted of selective examinations of procedures and representative

records, interviews with personnel and observations made by the inspector.

During the inspection, we also reviewed the circumstances associated with a

violation of transportation regulations involving a shipment of low specific

activity radioactive waste from your facility to Quadrex Corporation, Oak

Ridge, Tennessee, with holes in two of the drums in the shipment. Our findings {

f were discussed by Mr. Clemons with Mr. Sieber and other members of your staff

f: at the conclusion of the inspection on March 14, 1986.

In addition, three other violations of NRC requirements were identified during

.the' inspection. The four violations are described in the enclosed Notice.

Although Violation A, involving the shipment of two drums with holes in them

could be classified at Severity Level III, it has been classified at Severity

Level IV based on its low safety significance since (1) the amount of radio-

active material in each drum was minimal, (2) none of the radioactive material

was released from the drum, and (3) the drars were contained in an exclusive

use shipment. Nonetheless, these violations raise ccncerns regarding the need

l 'for improved control and supervision of such shipments in the future to prevent

j' similar violations. Any similar violations may result in additional enforce-

ment action.

You are required to respond to this letter, and should follow the instructions

specified in the enclosed Notice when preparing your response. In your

response, you should document the specific actions taKen and planned to prevent

recurrence. Further, you should describe the actions taken to improve

management control of the t.;nsportation program to prevent such violations in

the future. After reviewing your response to this Notice, including your

proposed corrective actions, the NRC will determine whether further NRC

enforcement action is necessary to ensure compliance with NRC regu1ctory

requirements.
