NOTICE OF VIOLATION AND NRC INSPECTION RFPORT
NO. 50-456/97022(DRP); 50-457/97022(DRP)

On January 26,1998, the NRC completed an inspection at your Braidwood Units 1 and 2 reactor
facilities. The enclosed report presents the results of that inspection. During the six week
inspection period, your conduct of activities was generally characterized by safety-conscious
operations, sound engineering and maintenance practices, and careful radiological work controls.
We are concerned, however, about a failure to take effective corrective actions, a failure to follow
procedurer., and a failure to adhere to design control requirements.
Three violations are cited in the enclosed Notice of Violation (Notice). The first violation involves

the failure to take effective corrective actions to address a previously identified design problem
with the Unit 1 and Unit 2 main feedwater check valves. The second violation involves the
failure, on five separate occasions, to follow station procedures for the control of maintenance
and test equipment. All of these instances were identified by the inspectors. The third violation
involves the failure to adhere to design control requirements before and after building temporary
structures in the auxiliary building. The failures to take corrective actions and follow procedures
have been identified in previous inspection reports and appear to be recurring problems.
Regarding Violations 1 and 2 described in the enclosed Notice relating to the failure to take
effective corrective actions and the failure to follow procedures, respectively, your response
should not only detail the actions you have taken or plan to take for the specific issues ident'Jied,
but should also be more broadly focused and address what actions you plan or have taker' to
ensure that your corrective actions process is effective. The NRC will use yc ur response, in part,
to determine whether further enforcement action is necessary to ensure compliance wits
regulatory requirements.
