Dear Mr. Hedger:
On January 6,1999, Ms. Neelam Bhalla of this office conducted a safety inspection at 345
North Pennsylvania Avenue, Wilkes Barre, PA of activities authorized under the license listed
l above. Your activities in NRC jurisdiction are authorized by a general license granted by 10
CFR 150.20(a). The inspection was an examination of your licensed activities as they relate to
radiation safety and to compliance with the Commission's regulations and the license
conditions. The inspection was limited to observations by the inspector, interviews with )
personnel, and a selected examination of representative records related to activities within the
NRC's jurisdiction. The inspection was conducted during the loading of Cobalt-60 sources into
the Elekta Instrument's Leksell Gamma Knife Radiosurgical unit. The findings of the inspection
were discussed with Cary Hedger of your organization at the conclusion of the inspection. The
enclosed report presents the results of this inspection.
