Inspection 50-333/86-01
This refers to the routine, safety inspection conducted by Mr. A. Luptak of
this office on January 18 to March 10, 1986, at the J.A. FitzPatrick Nuclear
Power Plant, Scriba, New York, of activities authorized by NRC License No.
.
OPR-59 and to the discussions of our findings held by Mr. Luptak with yourself
and other members of your staff at the conclusion of the inspection.
Areas examined during this inspection are described in the NRC Region I
Inspection Report which is enclosed with this letter. Within these areas, the
inspection consisted of selective examinations of procedures and representative
records, interviews with personnel, and observations by the inspector.
Our inspector also verified the steps you have taken to correct the violation
brought to your attention in the enclosure to our letter cated January 17,
1986. We have no further questions regarding your actions at this time.
Based on the results of this inspection, it appears that one of your activities
was not conducted in full compliance with NRC requirements, as set forth in the
Notice of Violation, enclosed herewith as Appendix A.
This violation has been
categorized by severity level in accordance with the General Statement of
Policy and Procedure for Enforcement Actions (Enforcement Policy),10 CFR 2,
-Appendix C (1985). You are required to respond to this letter and in pre-
paring your response, you should follow the instructions in Appendix A.
Apparently, the cause of the violation was the inadequate implementation and
control of surveillance testing, indicating a lack of supervision and
administrative control of the surveillance test program.
In addition, shift
personnel apparently did not realize that untested equipment is inoperable in
accordance with Technical Specification 4.0.C., indicating a misunderstanding
of the significance of a missed surveillance test.
Please address these
issues in your response to this letter.
The responses directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
