NRC INSPECTION REPORT No. 50-456/g7019(DRP); 50457/g7019(DRP) AND
Dear Mr. Tulw
On December 15,19g7, the NRC completed an inspecuon at your Braidwood Units 1 and :
reactor facilities. The enclosed report presents the results of that inspecuon. During the 6-week
period covered by this inspection, your conduct of acuviuos was generally choisoterized by
safety consc',us operations, sound engineering and ma!ntenance practices, and careful
radiological work controls. Operating crews poiforming the startup from the Unit 2 refueling
outage demonstrated excellent human performance when challenged by equipment failures.
Additionally, inspectors reviewed three safety-related modifloations that were installed during the
Unit 2 refueling outage and found that the modifications were processed in acemdance with the
appropriate procedures, were properly documented, and received proper safety evaluatioru .
How.sver, we are conoemed about the failure of station personnel to identify loose materials h
the Unit 2 containment that resulted in a violation. The inspedors found two exar. : es where the
noceptance critetta of me surveillance test procedure were not met following notifv tion by
a
station personnel that the containment loose debris inspecuon was complete.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violation are described in detail in the enclosed rsport. Please note that you are
required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. Your response should not only detail the actions to be
taken for the specific issues identified, but should also be broadly focused on what acGons you
plan or have taken to ensures that your corrective scuons process is effecuve. The NRC will use
your response, in part, to determir.e whether further enforcement action is necessary to ensure
compliance with regulatory requirements,
