REACTOR COOLANT PUMP SHAFT FAILURE ANALYSES

Reference:

FPC letter, Widell to Stolz, same subject, dated 5/5/86 As ycu know, the Office of Nuclear Reactor Regulation (NRR) has kept abreast 1,1986 reactor coolant pump (RCP) shaft of events related to the January (CR-3). Contacts between our staffs, with NRC failure at Crystal River Unit 3 resident inspectors and personnel at Region II, with other companies involved with the shaft and other pump components and their examination, and your presentation to us on April 24, 1986, have helped our understanding of the event and of your position in this matter as sunnarized in the reference letter.

You state that RCP shaft failure is bounded by the locked rotor analysis, and have provided additional clarifying information to the NRC staff in recent telephone discussions to support this conclusion. We agree with your conclusion.

Since a definitive root cause of the shaft failure has not yet been established, we agree that continuation of your thorough review of all relevant data is appropriate and necessary. Please continue to keep us apprised of new information and conclusions in this area.

In addition to the infonnation presented at the April 24th meeting and in the referenced letter, discussions between our staffs have indicated that you have installed an improved vibration monitoring system, including new calibrated sensors and readout and alarm capability.

In addition, you will continue to investigate state-of-the-art systems in order to permit utilization of an optimum system to assure early identification of any pump shaft damage so that appropriate corrective action may be taken before possible gross failure.

Engineering and operations staff as appropriate will be trained in the current system and any future modifications.
