FSAR page 9.2 5 states:'An automatic controlinterlock prevents the openin0 of the rendor plant component cooling water heat exchanger isolation valves when the isolation valves to the containment recirculation roolers are open."

Based on a review of the drawings the statement and drawings do not agree. The drawin0s Indicate that the Interlock prevents the opening of the 3SWP*MOV50 valves (Reactor Plant Component Cooling Heat Exchan0er Service Water Supply) when either of the associated 3SWP*MOV54 valves (Containment Recirculation Cooler Service Water Supply) is not fully closed. This interlock is bypassed, however, if both of the associated Service Water pumps are running.

The statement does not take into account the bypass option.

During the long term recovery of the socident, M la necessary to reopen the SSWPWOV50 valves and restore scryloe water to the FLPCCW heat exchen0ers for Spent Fuel Pool Cooling. To reopen these valves, the following lo060 is required

Note 1.3.10 of instruction PI 14, Revision 0, titled "Confl0uration Management Plan Project Process Administration instruction" states: "A CR will be -

generated to address issues of a repetitive or programmatic nature as determi ied by EP (Expert Panel). RP-4 'Corredive Action Program,' provides guidance on specific issues potentially -

affecting personnel safety, plant safety, or the proper operation of plant equipment that must be considered for a CR and brou0ht to the attention of the Shift Mana0er (SM). Where specific questions arise as to the need for a CR, EP will refer to RP-4 for additional Guidance."

The following examples of situations when a CR should be initiated are Diven in Attachment 3 to RP 4, Revision 4:

The CR's approved Corrective Action Plan (CAP), which is attached, was to have the Expert Panel re-review these specific UlRs and recommend any further action. The EP determined that no further action is required on any of these UIRs, for the reasons cited:

However, from the Millstone databases PDDS, EQ Master List, and the drawings Nos. 25212-21001 shi, and 25212 21004 shi, the location for motor operators 3SWP* MOW 1 A,B is EQ Zone AB-09. This is a discrepent condition.

The review of both EQ Jones show that t!i e environmental conditions are the same for AB-07 and AB-09 only during normal conditions only. During accident conditions, the maximum temperature in Zone AB-09 reaches 210 F and stays constant from 10 sec, to 2500 sec. after the accident.

The EQ conditions in Zone AB-07 is less harsh than AB-09. The expected maximum temperature in this EQ Zone AB-07 will not exceed 200 F , from 20 sec, to 30 sec, after the accident.

The review of the Limitorque motor orerator Test Report No.

B0056, Rev,0, dated 1/11/1960 for 3SWP*MOV71 A,B shows that the equipment is capable of maintaining its environmental qualification in both Zones AB-09 or AB 07.

The Procurement Specification for Safety Related Motor Operated Valves (MOVs) does not list the expected environmental conditions or the response spectra as required by the FSAR.

The Millstone Procurement Specification No. 2282.050-676 does not list the expected environmental conditions associated with the procured safety-related Class 1E motor operated valves (MOVs). FSAR Section 3.11B.2.2 require that all Class 1E equipment specifications define at least the worst case envelope service conditions anticipated for Olass 1E electrical equipment.

Also, the Required Response Spectra at the equipment locations are not provided in the specification per the requirements of FSAR Sections 3.10 and 3.118.1.1.

In LER 97 033-00 NU reported a non-conservative setting for the P-6 B! stable reset value which resulted in a condition where it is possible for the intermediate Range Neutron Flux, P-6 interlock to be bypassed and the Source Range Neution Flux monitoring instrumentation not re-energized and operable. In LER 97-045-00, NU indicated a discrepancy with regard to RPS response time testing. In all of these LERs the cause was indeterminate.

This DR is concemed with the fact that all of these LERs relate to inadequacies in RPS and ESFAS testing and, in the LERs, NU has not presented a discussion of the genericimplications of the eventa. Such a review is required by NU's corrective action process and the ICAVP would expect to see it reflected in the LERs.

NU has concluoad that the issue reported in DR.MP3 CS63 does not represent a discrepant condition. 3DBS-EDG 001 is the DBSD for the Emergency Diesel Engines. The operation of the ventilation fans is described in Section 6.0, Key System Parameters & 12.0, Component Fundions and Design Requirements, as supporting equipment. Their operation is described in the normal operating mode only as related to the operation of the EDGs. Comp!!ance whh Appendix R program requirements is a separate issue, and is adequately covered in numerous other documents, the conclusion reached in the DR was that the DBSD Description of EDG Supply Fan B operation is not complete.

Review of the DR and supportha documentation Indicates that the DR is worded incorrectly in that the 'A' f an start /stop/ auto logic contains the additional circuitry for operation from the fire transfer switch panel, not the *B' fan, as the DR states.

S&L reviewed PT 31470. The Secondary Containment Buundaries are outside the scope of this procedure.

Procedure U3 WC 14, On-line Scheduling, was to be revised to include a review of affected Technical Specifications with special attention paid to surveillance acceptance criteria. Prior to revision of WC 14 the commitment was revised. The revision deleted the requirement to review the surveillance of the acceptance criteria in a procedure. This requirement was judged by NU to not be practical r,or would it have prevented the event.

1. Rosemount Vendor Manual 656 0028 require that instruments w641 be mounted level. Flow transmitter SSWP-FT-500 is not installed level. Excessively out of level conditions may impact the accuracy of the flow transmitter causing mal-functions given the small pressure involved - this device measures differential pressures in the range of inches of water.

The difference in the elevation of the ports of th's device resulting from the out of-level condition could affect the ability of the instrument to accurately measure the pressure differential and, therefore, the flow in the pipe.

2. Helicoid /Ashcroft Vendor Manual 01M-226-002A rsquires that flush or v.all-mounted gauges are to be installed by means of screws. Pressure Indicators SSWP Pl139A&B,140 A&B,141 -

A&B and 142 A&B are all mounted with only one of the three required mounting bolts / screws.

3. Mounting and cormsion/ oxidation control requirements are set forth in Barton Manuals 01M-377 5A,01M-377.001B,01M-377.006A and Nu specificetions SP-EE-212-D-3 (2.23-2.25)

NETM-22 (2.27 & 2.28). Improper instrument mounting and lack of corrosion / oxidation control can lead to premature device failum.

- Differential pressure transmitters mounting screws & star washers on 3SWP*PDIS 24A, B, C, D, and pressure transmitters 3SWP-PDIS 110A, B, C, D located in intake structure / pump house show significant signs of oxidation / cor70sion. These instruments are exposed to a satt air atmosphere. The mounting screws, nuts and washers also may no longer meet seismic mounting and ASTM qualifications.

Insltrument 3SWP PDIS 1108 mounting screw does not have a star washer under screw head and no longer meets vendor mourtling instructions.

require that the components be solution annealled and water quenened. This predice produces non-sensitized steel. The performance of the solution annealling and water quenching of the spedfied components can be verified by the Certified Materials Test Reports ( See Attached CMTRs). This addresses issue number 1 of DR-MP3-0617.

Conclusion:
