ATTACHMENT BVPS-1 Report to IE Bulletin No. 85-03 Motor-Operated Valve Common Mode Failures During Plant Transients Due to Improper Switch Settings Each of the actions a through d stated in IEB No. 85-03 are addressed below. Report arrangement correlates to that of the actions in the Bulletin. This report is provided to satisfy the submitting requirements of Action e of the Bulletin.

a) Table 1 reports the results of the BVPS-1 review for motor-operated valves in the high pressure coolant injection system and the auxiliary (emergency) feedwater system. At BVPS-1 parts of the Chemical and Volume Control System, primarily the charging pumps, are also used for the high pressure coolant safety injection. Correspondingly, Table 1 includes the valves which function to isolate the Reactor Coolant System normal charging flow path and the valves required to align the charging pumps for high pressure safety injection.

A comparison of design differential pressures with accident system condition differential pressures in Table 1 (Columns D and H, respectively) confirms the adequacy of the original design specifications' criteria used to size the motor operators for these valves. The design differential pressures were determined from the applicable vendor's design specifications. The accident condition differential pressures were conservatively determined by assuming a single pipe failure downstream of the valve and applicable pump discharge or tank pressure at the inlet to the valve.

b) The design thrust requirements and the corresponding torque switch settings will be established for the design differential pressures identified in Table 1. These parameters will be calculated independently and verified against the valve operator manufacturer's original calculated data. The valve operator gear ratios will also be determined for compliance with stroke speed refer to Item C below for the method of I requirements, please

verifying the other switch setpoints.

c) The adequt.qy of existing valve operator torque levels, position limit switch set points, and protective overload capability will be verified by in plant testing using the motor operated valve analysis and test system (MOVATS). Duquesne Light Company has recently purchased this system and both operations and engineering personnel are trained to use this diagnostic equipment. The valves listed in Table 1 will be evaluated with this instrument and the switch settings will be changed if indicated by these evaluations. Normal post adjustment system testing and differential pressure testing will be performed to the extent required and permitted by system operation limitations. However, differential pressure testing should be minimal, since the valves were previously tested to demonstrate stroke time compliance at design differential pressure prior to shipment from the valve manufacturer.

Attcchment Page 2 d) The established open/close torque switch, maximum torque switch, torque bypass position limit, open position limit, and any specified stroke time requirements will be included in the BVPS-1 corrective maintenance procedure. These performance parameters will be required to be recorded in the "as found" and "as left" condition to assure compliance with the established performance parameters for the valves in Table 1.

e) The completion schedule is provided below. The completion date for the confirmation of switch settings occurs within the anticipated time of the sixth refueling outage for BVPS-1, which is currently scheduled to begin in November of 1987, to allow for operator switch testing as needed.
