REQUEST FOR ADDITIONAL INFORMATION - PREVENTIVE MAINTENANCE PROGRAM FOR CONTROL R0D DRIVE AND ORIFICE ASSEMBLIES We are reviewing your submittal dated February 3, 1986 (P-86078) proposing a new preventive maintenance program for the control rod drive and orifice assemblies (CRD0A) at Fort St. Vrain (FSV). We find that in order to complete our review additional information is needed as listed in the enclosure to this letter. Please provide this information within 45 days of the date of this letter.

The information requested in this letter affects fewer than 10 respondents; therefore, OMB clearance is not required under P.L.96-511.

, PROGRAM FOR CONTROL ROD DRIVE AND ORIFICE ASSEMBLIES FORT ST. VRAIN (TAC 61066)

PLANT, ELECTRICAL, INSTRUMENTATION AND CONTROL SYSTEMS BRANCH The maintenance program for the Control Rod Drive and Orifice Assemblies (CRD0A) described in your submittal (letter dated February 3, 1986) lists several activities which should be performed in order to keep the CRDOA in operating condition. However, no details are given of the exact nature of these activities. Particularly, the acceptance criteria for the inspections specified in the program are missing. Please augment the CRD0A preventive maintenance program to include the information requested below and submit the requested information for our review.

(1) Please provide the acceptance criteria for the visual inspections of the following components listed in your proposed preventive maintenance program:

o pinion gear and brake pads in the shim motor / brake assembly o drive. train o cable o hopper and samples of materials from the reserve shutdown system o orifice drive motor assembly o lower seal o primary seal - 600 assembly o primary seal - 200 assembly (2) Please describe the nature of the analyses and the acceptance criteria applied to the results of these analyses for the following items:

o surface wipes of the cable o materialsfromthereserveshuttpwnsystem (3) Pleas *eprovideacceptancevaluesforkhemeasuredtorquesinthe foll6 wing components:  :
