NRC INSPECTION REPORT NO. 50-461/97022(DRP) AND NOTICE OF

VIOLATION

Dear Mr. Cook:

On November 25,1997, the NRC completed an inspection at your Clinton Fac:lity. The

enclosed report presents tha results of that inspection.

During the period covered by this inspection, improvements were noted in the quality of the

audits conducted by your quality assurance personnel, in addition, hydramotor work was

stopped by workers when procedure revisions were needed. However, continuing examples

,

of the failure to thoroughly evaluate, properly p-lorltize and effectively correct identified

problems were noted. For example, a narrowly focused review of the 480 volt motor over-

gressing !ssue led to a 7 week delay in performing an operability determination for these

motors.

,

In addition, we are concemed about three violations of NRC requirements that were

identified, in the first violation, Technical Specification required actions when a limiting

condition for operation was not met were not implemented on two occasions. On the first

occasion, improper verification of an attemate decay heat removal source resulted in not

identifying that a valve had been mispositioned for approximately 3 months. On the second

occasion, actions were not pursued to resolve 4160V breaker deficiencies without delay.

This violation is of concern because operations personnel were unaware that Technical

Specification required actions had not been performed,

in the second violation, NRC inspectors idr/ofied that vendor manuals supplied with

maintenance and test equipment were not controlled as required by 10 CFR Part 50,

. Appendix B, Criterion VI. Specifically, your procedure for processing vendor information did

not require measurement and test equipment vendor manuals to be controlled documents

even though the manuals were used in the performance of work activities. This violation is

of concern because it represents a continued problem with providing controlled documents

for maintenance and surveillance activities,

In the third violation; NRC inspectors identified that work instructions for repairing safety

related hydramotors were inconsistent and not prepared as required by procedure.

' Although a specific procedure for the repair and overhaul of hydramotors existed, the

maintenance work package allowed the work to be performed under broad work instructions

'

- and enclosed the specific procedure as a reference document. This violation is of concem

because detailed work
