This is in reference to your letter dated June 6, 1985 requesting termination of NRC License Number SNM-1393.
Per a telephone conversation between Jack Dagenais and B.J. Holt on November 19, 1985, it is our understanding that the above mentioned letter was written in error. You do not wish to terminate your license authorizing the use of plutonium in cardiac pacemakers because all of your units have not been recovered. However, you do wish to amend your license for inclusion of a physician, Wayne Breece, M.D., who will be responsible for the explantation, recovery and disposal of remaining pacemakers.
In order for us to review this request, we will need the following information on Dr. Breece:
1.
His specialty, board certification, and position at Hurley Medical Center.
2.
A description of his experience in the follow-up and explanation of pacemakers, including specific information on the numbers of pacemakers followed and/or explanted.
3.
Confirmation that Dr. Breece is familiar with and understands the standard research protocol developed by Medtronic describing the follow-up program for patients in whom the pacemakers have been implanted.
Please reply in duplicate, within 30 days and refer to Control No. 25862.
