Dear Mr. Holt:

My apologies for the delinquent nature of this letter.

I have been asked by liurley Medical Center to represent that institution, with respect to this license.

At one time, I had three patients in my practice who had reccived a nuclear pacemaker by Doctor William Weber, now deceased. One patient has subsequently died and this pacemaker has been removed by the coroner and disposed of by MedtronMs.

Most recently, I have removed a unit myself and, again, Medtronics supplied the shipping material and was responsible for disposing of this unit.

I currently have a single patient with a permanent pacemaker of the nuclear variety. I am following the protocol set forth by the Medtronics Corporation for following this unit on a monthly basis. I am also familiar with the protocol for removing and disposing of this type of unit.

I am a Board Qualified Cardiologist and received my cardiology training at Baylor College of Medicine. I have not implanted any nuclear pacemakers myself; however, have considerable experience (over 150 implants of the standard pacemaker units).

Once our remaining nuclear pacemaker has been properly disposed of, we have no further plans for continuing the use of nuclear pacemakers in the future.
