Escutcheons for the switchgear Local / Remote Selector Switch and M81 Start /Stop Switch are missing from Logic Diagrams LSK 9108, Rev 12 and LSK 9-10J.

Ducription: FSAR Section 6.5.2.1 identifies the containment recirculation fluid to have a pH of 7.0 to 10.5. However, the containment recirculation pump design specification 2214.802 044 through Revision 1 shows the pump to be designed for a pumped fluid pH renge of 5.0 to 7.5.

According to specification 2214.802 044, the quench spraf pump wetted pads are made of Type 304 stainless steel. These materlats are suitable for the design ph range of 7.0 to 10.5.

1. Drawing EE 6EL Rev. 2 and the Cable and Raceway Program (TSO2) indicate conduit number 3CK949ND9 runs between 3 HTS-JB982P and 3 HTS-JB980S. The conduit is tagged 3CK949NB9 in the field.
2. The ground wire running with the flexible conduit connecting to 3QSS-FIT 22 is not connected at the non instrument end.

Grounds for flexible conduits are required to be connected per ElectricalInstallation specification E 350.

The following material condition was noted;this is not a configuration issue:

1. The conduit connection fittings to boxes 3 HTS-JB979S and JB924S are very rusty.
2. The instruments (RTDs/TEs) of the heat tracing system are not tagged contrary to requirements of ElectricalInstallation 3pecification E 350.

Date Published: 11/2/97 Duropency: Cable Routing is Not Consistent With TS02 Deecription: Conduit 3CK768PB-4' runs between MCC 3EHS*MCC184 and the tray system above the MCC providing transition of cabling to the supplied components. There are three "K" service level 480 V power cables in this condult. These cables exit sis conduit with at least one of the caules entering a tray marked in the field as 3TC768P - a 120V control ("C" service level) cable tray.

This marking appears to be consistent with the tray arrangement drawings. The TSO2 routings for all three of these cables show that these cables are all routed into 3TK768P - a 480V power cable tray. The routing of higher energy cables (480V) in lower energy trays is generally not done, The Cable / Raceway Control Program Users Manuals indicates that "C" cables may be routed in "K" trays on a limited basis, but does not allow 'K" cables in

'C' trays.

The routing of power cables in control trays could adversely affect all control circuits with cables routed in the tray in the event of the power cable / circuit malfunction. Such a condition has the potential to render several components of multiple systems within the train inoperable.

2. The Cable ano Raceway Program (TSO2) lists conduit 3CC880PD vith *from ties' of both tray 3TC880P and tray 3TC881P. The installed conduit is clearly associated tray section 3TCS81P which in tum is connected to tray 3TC880P.
3. The ground wires running with the raceway terminating flexible conduits for valve SWP*MOV24C are not ennnected at both ends as required by ElectricalInstallation Specification E-350,
4. The ground wire running with the raceway terminating flexible conduit for SWP TE127C is not securely conn 3cted at both ends as required by Electrical Installation Specification E 350.

The following material condition was noted; this is not a configuration isse.

1. Conduit 3CC881PH connects to the motor space heater terminal box 3SWP'TBPID2 via a flexible section of conduit.

This flexible conduit was observed to not be property connected exposing the cabling.

This would prevent the temperature controller from modulating the inlet, outlet and recirculating air dampers that would be required when only one supply fan is operating.

Motor Operator Replacements for Valves 3QSS*MOV34A and 3QSS*MOV34B Motor operated valves 3QSS*MOV34A and 3QSS*MOV34B were changed from the original purchase specification from 0.13 to 0.33 horsepower. Neither Specification 2382.200-184 Add.1 nor Production Maintenance Management System (PMMS) have been revised to reflect the changes in horsepower, motor torque, full load current, and locked rotor current to valves 3QSS*MOV34A and 3QSS*MOV348. These documents should be revised. The affected voltage and thermal overload sizing Calculation 89 094120E3 (Rev. O, CCN 4) has evaluated both horsepower values, therefore the results of the calculation are not affected.

B. Reliance Motor Curves for Valves 3QSS*MOV34A and 3QSS*MOV34B 1, Fullload current can be obtained from the Reliance motor curve sheet (included in the motor operated valve calculations) in three places: the table, the header, and the curve itself. The values from these three places are usually different. In most cases, the motor operated valve Calculation 89-094120E3 (Rev.

O, CCN 4) states that the full load current is obtained from a walkdown reading of the nameplate, therefore the full load current data shown in the Reliance motor curve sheet is not used. However, for valves 3QSS*MOV34A and 3QSS*MOV348, the thermal overioad heater analysis section of the calculation states that the full load current value was obtained from the Reliance motor curve sheet, but no indication is made to whether the table, the header, or the curve itself was used to obtain this value. The different fullload current values are:

Reliance Motor Curve - curve = 0.73 amperes Reliance Motor Curve - header = 1.0 amperes Reliance Motor Curve table = 0.7 amperes Value used in the thermal overioad relay sizing section of the calculation = 0.73 amperes.

The use of 0.7 amperes has no affect on the calculation. The use of 1.0 amperes reduces the actuation time of the thermal overload relay but the results of the calculation are not changed, Printed 10/31197 to 01:10 AM page 1 of 3

Calculation 89-094-1RE3 uses different full load current values (0.73 or 1.0 amperes) in 'he AC Motor Evaluation Checklist, Limitorque EEQ Walkdow,' Check!!st, motor overioad test record, and molded case circuit breaker test record sheets, in add!'lon, the full load current values are different between the calculation, vendor Drawings 2382.200-164 -043 (Rev. C) and '

2362.200-164-043A (Rev. B) (0.95 amperes) and Plant Design Data System (PDDS) (1.0 amperes). These documents should be revised to reflect the actual full load current.

2. For motor operated valves 3QSS*MOV34A and 3OSS*MOV348, the Reliance motor curve in Calculation 89-094-120E3 (Rev. O, CCN 4) indicates a locked rotor current value of 5.5 in the header of the curve but a locked rotor current value of 5.25 in the table of the curve. The calculation doesn't address which value is u ed or why it is used. In addition, Calculation 89-094120E3 does not match the values shown in vendor Drawings 2362.200164-043 (Rev. C) and 2362.200-164-043A (Rev. B)

(5.0 amperes) and PDDS (5.8 amperes). The adual value of the locked rotor current should be used in these documents.

Calculation 89-094-120E3 used the value of 5.5 amperes in the thermal overload relay sizing and in the overload sections of the calculation. This provides more conservative results than using 5.25 amperes.

C. Motor Ampacities of Valves 3QSS*MOV34A and 3QSS*MOV348 For motor operated valves 30SS*MOV34A and 3OSS*MOV348, Production Maintenance Management System (PMMS) has three ampacity categories and values for two ampacity attributes:

AFL = 0.39 ALR = 2.6 Amps = 0.13 Comparing the values in PMMS with other documents, "AFL" and "ALR" represent full load and locked rotor current, respectively, it appears that " Amps" represents the old horsepower of 0.13. " Amps" should be revised to reflect the actual motor attribute and value.

Celculation 89-094 120E3 is used to size the motor thermal overloads and confirm adequate voltage exists at the motor's terminals. The stroke time in Calculation NM-027 is 40 seconds for these valves. The stroke time in Calculation 89-094-120E3 is 30 seconds and the stroke time in Specification 2362.200-164 Add.1 is 30 seconds (maximum). The 40 second stroke time does not affect the results of the calculation (i.e., the thermal overload relay will not actuate within 5e duty cycle of the valve actuator motor).

In the thermal overload analysis section of Calculation 89-094-120E3, it references Calculation NM-027 as the input source for stroke time. Calculation 89-094-120E3 references Revision 1 of Calculation NM-027, but the latest revision of NM-027 is Revision 2. The latest revision of NM-027 should be referenced.

2. For each motor operated valve, the header on the Reliance motor curve iefers to the insulation as "B" which does not agree with the purchase specifications which require an insulation rating of radiation resistant Class H.
3. In Calculation 89-094-120E3 (Rev,0, CCN 4), the locked rotor current for valves 3RSS*MOV20A,3RSS*MOV208, 3RSS*MOV20C, and 3RSS*MOV200 t sries between 5.25 amperes as shown in the table on the nellance motor curve M2734 (dated 7/25/77) and 5.5 amperes which is shonm on the undervoltage analysis, thermal overload heater analysis, AC motor evaluation checklist, and thermal overload relay checklist of the calculation. However, Calculation 89-094-120E3 only uses the 5.5 ampere value throughout its analyses (i.e., is consistent). This value results in a more conservative but acceptable voltage drop than when using 5.25 amperes. The use of either value meets the requirement that the thermal overload relay actuate before reaching the motor capability.
4. In Calculation 89-094-120E3 (Rev. O, CCN 4) for valves 3RSS*MOV20A, 3RSS*MOV208, 3RSS*MOV200, and 3RSS*MOV200, the Reliance motor curve shows a horsepower rating of 33 and the Limitorque EEQ Walkdown Checklist, motor overioad test record, and molded case circuit breaker test record sheets of Calculation 89-094-120E3 show a horsepower of 0.125 which does not match the value of 0.33 horsepower used in the undervoltage and thermal overioad heates analysis of the calculation. These documents should be revised to reflect the actual motor horsepower.
5. Fullload current can be obtained from tne Reliance motor curve sheet (included in the motor operated valve calculations)

However, for valves 3RSS*MOV38A, 3RSS*MOV388, 3RSS*MV8838A, and 3RSS*MV88388 the calculations state that the full load current value was obtained from the Reliance motor curve sheet, but no indication is made to whether tha table, the header, or the curve itself was used to obtain this value. The different full load current values are shown below for these valves:

O CCN 4) have not been revised to reflect the changes in horsepower, motor torque, full load current, and locked rotor current to valves 3RSS*MOV20A, 3RSS*MOV208,3RSS*MOV20C, and 3RSS*MOV200, even though these valves were replaced in 1985. The latest change notice (CCN #4) still reflects these pages as valid (i.e., they are not noted as being void nor is there a statement that they will be replaced in the future). Both PDDS and PMMS refer to the replacement valves as being provided by Specification 2362.200-164 Add.1 (same as the originally purchased valves), but this specification still references the horsepower, full load current, locked rotor current, and torque of the originally purchased valves even though the vendor drawings reflect the horsepower of the replacement valves (0.33 HP). PDDS has not been revised to reflect the change in the valves' horsepower, full load current, and locked rotor current. The vendor drawings reference the old torque value (2 ft-Ibs) and different full load and locked rotor currents than the calculation or PMMS.

Calculation NL-038 (Rev. 2, CCN 6) references the old motor (0.13 horsepower and 3.15 locked rotor amperes). These documents should be revised to reflect the latest motor data.

Section 3.5 (Load Data Table), the BHP used for the QSS and RSS pump motors is the rated or normal BHP (380 and 450, respectively) but the BHP for the SWP pump motors is the maximum BHP (561). In Calculation SP M3 EE 342, Section 4.9 (Motor Data Log), the BHP used for the QSS and RSS pump motors is the maximum BHP (470 and 458, respectively) but the BHP used for the SWP pump motors is the rated or normal BHP (555). Since the actual brake norsepower of 3SWP*P2A,3SWP*P28, 3SWP*P3A, and 3SWP*P3B is less than thet assumed in the calculation, the results of Calcu!ation SP M3 EE 342 are conservative. The data listed above for SSWP*STR1 A, 3SWP*STRIB,3SWP*STR10, and 3SWP*STR1D have no effect on the results of the calculation.

Since the actual motor data is available, either Calculation SP-M3-EE 342 should be revised to reflect this data or the statement in Appendix B Page B2 of Calculation SP M3-EE 342 should be revised.

2. There is a discrepancy in the minimum required voltage for 3SWP*TV35A and 3SWP*TV358. Calculation 209E (Rev. O, CCN 2) Page 8 states that the minimum required voltage is 97 volts (this value is discussed in the telephone memora., "um attached to Calculation 209E as Page 79). The vohage used for MCC 3EHS*MCC182 (which feeds 3SWP*TV358) is shown as 430 volts (Page 8 of the calculation), but Degraded Voltage Calculation NL-042 (Rev. 2 CCN 5) now shows this MCC to have a voltage of 424.8 volts. Using the NL 042 MCC voltage results in a motor terminal voNage of g9.6 volts, which satisfies the minimum voltage requirement of 97 volts shown on Page 6. l l

The minimum calculated voMage for motor 3SWP*TV35A is l 102.5 volts as shown in Calculation 210E Page 86. The voltage l used for MCC 3EHS*MCC1 A2 (which feeds 3SWP*TV35A) is shown as 434 volts (Page 6 of the calculation), but Calculation NL-042 now shows this MCC to haw a voltage of 427.9 voHs. I Using the NL 042 MCC voMage results in a motor terminal l voMage of 96.4 volts, which does not satisfy the minimum l voltage requirement of 102 volts shown on Page 86 of l Calculation 210E nor does it satisfy the minimum voltage I requirement of 97 volts shown on Page 6 of Calculation 209E.

Note that Calculations 209E and 210E state that the calculation of record that determines the MCC bus voltages is NL-038. It appeam that Calculations 209E and 210E should be revised to use Calculation NL-042, which provides lower minimum voltages and is more conservative than Calculation NL-038, and not NL-038 for determining the MCC bus voltages.

3. For motors 3SWP*P1 A,3SWP*P18,3SWP*P1C, and 3SWP*P10, Calculation GM-60-03.405CA (Rev.1) shows an acceleration time of 6 seconds for a minimum starting voltage of 70% of rated voltage. This does not agree with the 5 second acceleration time shown in Specification 244t003-009 (Rev. 2)

1. Support mark number #111 is shown as type R203A on drawing EE 34EE Rev. 6. The drawing does not list R203A in table of support types and the Cable and Raceway Program (TSO2) lists R203 for trays supports for trays 3TX 014N, 3TK007N and 3TC 013N, which includes support #111.
2. Drawing 12179-EE 46D, Rev 13, shows put! box '3JB 0161*

located in the containment at elevation 34 feet (drawing coordinates 1-6). This box is actually 3JB*0161. The non safety related box (3J3-0161) is located at elevation .18 feet.

3. Note 2 of Drawing, Indicates that all trays are 30 inches wide unless noted otherwise. Trays 3TC0014N,3TC0015N, and 3TX016N are shown on this drawing and without a commodity type indicating a different tray size displayed for these trays.

TSO2 indicates these trays are 18 inches wide The trays are depicted on the drawing similady to trays that are listed as 30 inches.

Date Published: 11/2/97 D6 crepancy: Conduit Support Differences Ducription: 1. While checking supports CS 7503 and CS 7445,it was noted that the conduit fitting cover for 3CX900PB4 was missing. The fitting's open face is upward. Location is near CS 7445.

2. An unidentified / unscheduled 2' con 6uit, which according to Conduit Support Log 12179-FSK-CS 3348, Rev. 6, is supported by support CS 3348,is not clamped on support CS-3348. The resulting span is over 10' 6* which exceeds the allowable span of 8' 0* per BE 52AN Rev. 5 and AU Rev. 4..
3. Field inspection of support R208A-098 found a section of 3/4 inch Steel conduit that is not clamped to the support, but is resting on conduit supported by R208A-098. This conduit is in need of support in this approximate location to meet span critoria and to take credit for the tray support supporting the conduit, a clamp / attachment is necessary.
4. Conduit Support Log (CSL)12179-FSK-CS-1343, Rev. 7, with no outstanding change documents, lists conduits 3CX900PA, 3CX900WB2, and 3CX900PC2 as supported by this support.

These condults are not included in TSO2 as supporied by this support. TSO2 lists an additional condult,3CX900PC, not shown on the CSL, as supported from this support.

5. Conduit Support Log (CSL)12179-FSK-CS-3058, Rev. 5, with no outstanding change documents, lists conduit 3CX900PA4 as supported by this support. This conduit is not included in TSO2 as supported by this support. TSO2 lists an additional conduit, 3CX900PA, not shown on the CSL as supported from this support.
6. Conduit Support Log (CSL) 12179-FSK-CS-4798, Rev. 2, without outstanding change documents, indicates that conduit 3CX9000B4 is supported from support CS-4798. TSO2 also indicates this conduit is supported by the support. The detail shown on the draw lng is for a field installed junction box and does not include any attachment of condult. Further. the conduit is installed in the field with two supports, CS-7482 and 7483 which are listed in TSO2 and the CSLs correctly.

3. Field inspection of support R150-055 found that the bottom tray shelf member is insimited at a 45 degree angle for proper fit-up to tray. This is not noted on the detail drawing EE 34HA Rev.

6 and is not covered by any listed open document.

4. Drawing EE 34T Rev. 1, indicates that all tray covers for instrument trays shall be flat. This is consistent with the tray detail drawing EF-34MJ Rev.3 and tray cover identification drawing EE 34TQ Rev. 2 which indicates top and bottom covers. The Cable and Raceway Program (TSO2) indicates that at least one of the covers for tray riser 3TX1n4P is vented. Field observation of the tray identified a vented cover on the bottom and flat on the top.
