Gentlemen:

SUBJECT: NRC INSPECTION REPORT NOS. 50-321/86-14 AND 50-366/86-14

This refers to a special Nuclear Regulatory Commission (NRC) inspection which

was conducted by Mr. D. Thompson on April 17-18, 1986. This ir,spection included

a review of activities authorized for your Edwin I. Hatch Nuclear Facility. The

purpose of this inspection was to review your investigation and verify your

corrective actions regarding a failure to control access to the protected area,

which occurred on April 15, 1986, and was reported telephonically to Region II by

the Nuclear Regulatory Commission Resident Inspector on April 16, 1986.

The findings of this special inspection, which are documented in the enclosed

report, indicated that the security degradation resulted from a violation of

regulatory requirerents. These findings were discussed with Mr. Harvey Nix,

General Manager, on April 18, 1986.

The inspection findings indicate that certain activities appeared to violate NRC

requirements. The violation, references to pertinent requirements, and elements

to be included in your response are described in the enclosed Notice of

Violation. The violation involved failure to provide positive access control to

a protected area. After careful evaluation of the circumstances surrounding this

violation, which included such considerations as the short length of time the

individual was alone in the protected area and the fact that the individual was

cleared, authorized, and properly logged into the area, we have concluded that

the violation is appropriately categorized as a Severity Level IV.

Notwithstanding the above conclusion, we believe this matter deserves special

management attention to assure that supervisors and members of the security

force, and other personnel comply with established security procedures to ensure

positive control of access to protected areas. Additionally, as you know there

have been previous events related to improper access controls or inadequate

search of personnel entering the protected and vital areas. We believe the

configuration of Hatch access control facility is inadequate which precludes an

orderly flow of personnel through the access portal during high traffic periods.

We noted in your July 5,1984, response to a similar violation you stated, " site

and corporate management are in the process of reviewing the Plant Hatch security

facilities and will pursue improvements as necessary which are expected to b

improve security effectiveness, and thereby minimize the possibility of events of

'

this type in the future."

It appears with the occurrence of ancther similar event, the improvements under

taken may not have been adequate to minimize the possibility of this type event

from recurring. Accordingly, in preparing your required response, you should

include a description of the. preventive measures derived from an evaluation of

. the underlying causes of this event, specifically an evaluation of the adequacy

of the Hatch access control facility. Your reply and the results of future

inspections will be considered in determining whether further enforcement actions

are appropriate.

In accordance with 10 CFR 2.790(d) and 10 CFR 73.21, safeguards activities and

security measures are exempt from public disclosure; therefore, the enclosures to

this letter, with exception of the renort cover page, which presents a nonexempt

summary, will not be placed in the NRC Public Document Room.

clearance procedures of the Office of Management and Budget as required by the
