ENCLOSURE 1 NOTICE OF VIOLATION Duke Power Company Docket No. 50-369 McGuire License No. NPF-9 The following violation was identified during an inspection conducted on April 7-11, 1986.
The Severity Level was assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).
Technical Specification 4.0.5 for McGuire Units 1 and 2 requires that
" inservice inspection of ASME Code Class 1, 2, and 3 components and inservice testing of ASME Code Class 1, 2, and 3 pumps and valves shall be performed in accordance with Section XI of the ASME Boiler and Pressure Vessel Code and applicable Addenda as required by 10 CFR 50, Section 50.55a(g), except where specific written relief has been granted by the Commission pursuant to 10 CFR 50, Section 50.55a(g)(6)(i)."
The McGuire pump and valve inservice testing program was developed in accordance with the 1980 Edition of the ASME Code.
IWV-3413, (b) of the Code requires that the stroke time of all power operated valves be tested on an interval specified by IWV-3412.
IWV-3417 requires that any abnormal or erratic action observed in the stroke timing shall be reported so that corrective action can be initiated immediately.
Contrary to the Code requirements, test data documented during the first three years of Unit 1 operation for valves ND 34 (RHR System) and NI 332A (CVCS) indicated stroke times of 0.0 seconds.
Identical valves in Unit 2 had measured stroke times of approximately 17 seconds and 10 seconds respectively.
No evaluation of this erratic behavior was documented or reported.
This is a Severity Level V violation (Supplement I).
Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
