Limerick Generating Station Unit 1 Annual Licensed Operator Requalification (LOR) Program

Dear Mr. Butler:

The 1985 Licensed Operator Requalification (LOR) Program was reviewed by NRC Division of Licensing and Philadelphia Electric Company (PECo) was requested in a letter from Walter R.

Butler (NRC) to E. G. Bauer (PECo) (dated September 29, 1985) to provide additional information and clarification.

The attached Enclosures 1 and 2 fully describe the plan for the 1986 Limerick Generating Station (LGS), Unit 1 LOR Program and address the request for additional information and clarification which the NRC requested as a result of the review of the 1985 LOR Program.

Several changes are contained in the 1986 LOR Program when compared with the 1985 LOR version; these include the following:

1. Licensed Operator / Senior Licensed Operator (LO/ SLO)

Required Reading List Information will be added to each training cycle to cover the previous 2 months of Required Reading Material.

2. No LOR training is scheduled to be conducted during the period of June 23, 1986 and September 1, 1986 in order to allow for LO/ SLO vacations.
3. Annual requalification examinations will be given twice during the period of the 1986 LOR. Fifty percent (50%) of the LO/ SLO personnel will take their annual exam after the completion of Cycles 1 through 3 in November 1986. The remaining 50% of the LO/ SLO personnel will take their annual LOR exam in March, 1987 covering the subject material of the entire 6 cycles. This will avoid examining the entire LOR group at the end of each year's requalification and thereby reduce the disruption to the shift scheduling resulting from any retraining or re-examination which may be required.
4. Sixty-one (61) LO/ SLO personnel and seven (7) to eleven (11) Shift Technical Advisors (STA's) will participate in the 1986 LGS LOR Program. In order to ensure optimum instructor to student ratio for simulator training, each class will be limited.

This will require assignment of SLO engineers and those STA's not on-shift to specific weeks of each LOR cycle in an effort to limit each class to the optimum rate.

The 1986 LOR Program will satisfy the requirements of the LGS FSAR; ANSI 3.1-1978; NUREG-0737; NUREG-1021; LGS Tech Specs para. 6.4.1; 10CFR55 Appendix A; and LGS Administrative Procedure, (A-50) " Conduct of Station Training".

The 1986 LOR Program is the second year of a 2-year cycle of LOR subjects. It will consist of 6 cycles of information which will include a mix of classroom, simulator, and plant tours. Additionally, those items which where found during the 1985 LOR examination process to require reinforcement will be factored into the 1986 LOR schedule as appropriate. This ensures healthy feedback to re-discuss those items which past experience has shown to require additional discussion for Licensed Operator, Senior Licensed Operator and Shift Technical Advisor personnel.

ENCLOSURE 1 LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM The Licensed Operator Requalification Program shall comply with the requirements of the LGS FS AR, ANSI 3.1-1978 (which is the ANSI version presently committed to in LGS FS AR), NUREG 0737, LGS Technical Specification paragraph 6.4.1, and 10CFR55 Appendix A.

These documents are used as required to define the content and enhance the quality of this training program. This requalification program shall address the following areas:

A. Schedule for implementation

1. Lecture series
2. On-the-job training / Reactivity Control Manipulations
3. Design, Procedure, and License Change Review
4. Procedure Review B. Evaluation of Per formance C. Records D. Quality Assurance A. Schedule The Licensed Operator Requalification Training Program for licensed operators, senior licensed operators, STA's and SRO-Certi fied ins tructors shall be es tablished and ready for implementation no later than three months following issuance of the station operating license. The program shall be conducted on a two year cycle and shall be followed by successive two year programs which meet or exceed the requirements of 10CFR, Pa r t 5 5, Appendix A as described below. The program, which also takes in to consideration holders of conditioned licenses (such as for fuel handling only), consists of the following major componen t s :
1. Lecture Series The requalification program shall include preplanned lectures on a requiar and continuing basis. Annual written examination results will indicate the scope and depth needed. Lectures cover the following areas:

Reactor Theory and Principles of Reactor Operation General and Specific Operating Characteristics of the Plant Instrumentation and Control Systems Reactor Protection and Engineered Safety Systems Normal,, Abnormal, and Emergency Operating Procedures Radiation Control and Safety; Radioactive Material Handling Fuel Handling and Core Parameters

Licensed Oparator Raqualification Training Program Page 2 of 14 Administrative Procedures and Technical Specifications Applicable Portions of Title 10, Chapter I, Code I of Federal Regulations

- Nuclear Power Plant Design Features Mitigation of Accidents Involving a Degraded Core

- Emergency Plan Thermodynamics, Heat Transfer, and Fluid Flow Films, videotapes, and self-study material may be used to supplement the lectures. Lectures will be scheduled at a rate of not less than six per year, appropriately spaced through the year and taking into consideration expected personnel availability and plant operations. A minimum of 12 lectures for each license cycle ( i.e. , 2 year period) shall be scheduled . Lectures may be deferred due to unexpected plant operations; however, these lectures shall be conducted as soon as practicable thereafter.

Individual study on the part of each operator shall be encouraged. For the case Where an individual trainee misses an assigned lecture or classroom weekly session, that LOR participant shall be required to attend another scheduled lecture during the 6-week cycle or self-study prior to taking the required quiz for the weekly classroom session. All LOR participants must take and pass (with at least an 80%

grade) the weekly quiz for each cycle of the LOR program.

The method of determining required attendance at the classroom -lecture topics is defined as follows:

(1) All LOR participants (LO, SLO, STA, and SRO-

, Instructor Certified personnel) shall normally-l attend each weekly cycle of the LOR program for l the year; the only exception to this is for those LOR participants Who in the past year's annual requalification exam received a grade of 80% or greater in the topic and they answered all questions relating to important safety aspects of a critical system of that topic correctly (i.e.,

>90% grade as well on those particular questions).

It should normally be the policy to have all LOR participants attend all LOR topics every year regardless of their previous year's LOR exam

. grades. Normally, only in the case of impingement on plant operations will the exemption of >80%

grade in a topic allow an LOR participant to miss an assigned LOR classroom session.

r Licensed Oparator Rsqualification Training Program Page 3 of 14 (2) All STA participants shall be examined in the classroom tecture series on the SRO level of knowledge and are administered the SRO LOR yearly exam as a means to maintain their retention of SRO topics.

(3) All SRO-Certified Instructors shall either be:

- tat actively involved in the classroom tecture series either by attendance at the LOR session itself, instruction of the LOR session itself, development of the less plan materials for that LOR session, or (b) actively involved with comparable attendance, instruction, or development of similar topic / subject matter in either the initial STA or initial hot license (RO or SRO) training programs. In the case of (b) above the LGS Training Coordinator will decide if program attendance is considered both pertinent and/or active. This participation in a program other than U3R for Which subject matter credit is being taken should be addressed by written memo or attendance files in the training record of that individual SRO-Certified Instructor.

(4) For the case of LOR participants (as discussed in (1) above)' attending a non-mandatory LOR weekly lecture session and then failing the end of lecture quiz, that LOR participant will be given remedial training or self-study as appropriate in the topic / subject matter f ailed. The LOR participant will then be required to take and pass (with >80% grade) a dif ferent quiz covering the previoUsly failed subject matter.

(5) The LGS Training Coordinator in conjunction with the LGS SRO Licensed Senior Plant Staf f (i.e. I Operations Engineer, Technical Engineer, etc. h shall make the decision to determine required attendance of all personnel in the LOR classroom lecture series from year to year. This ensures -

both plant and training input determine Whether a particular licensed operator, STA, or. instructor have to attend all sessions.

During the course of the Lecture Series, written quizzes (normally weekly) shall be administered. For those Whose attendance was mandatory, a grade of less than 80% will require retraining and ratesting in the subject matter covered. All LOR participants are required to take and pass all weekly LOR cycle quizzes for Which they are requireN Eo attend. This is a

Licensed Operator Rrqualification Training Program Page 4 of 14 prerequisite for taking the annual LOR exam to maintain a license or certification.

2. On-the-Job Training / Reactivity Control Manipulations During the term of Ehe license, each licensed operator is required to manipulate the plant controls through at least ten (10) reactivity changes and each senior licensed operator is required to manipulate, or direct the manipulate of controls through ten (10) reactivity changes.

The requalification program shall ensure that each licensed individual per form or participate in an acceptable combination of reactivity control manipulations consistent with plant and personnel availability and scheduling. Credit for these activities will be limited to one or two licensed operators performing the task and one or two senior licensed operators engaged in directing the operations.

A simulator, such as the Limerick Training Center Simula tor, which reproduces the general operating characteristics and has similar instrument and control arrangement to Limerick Generating Station, may be used to perform these reactivity control ma nipula t ions . All licensees shall participate in simulator programs as part of the requalification program. Simulator Exercise Guides ( SEG's) and Simulator Training Scenarios (STS's) shall be utilized by all qualified simulator instructors when administering training or evaluations concerning required plant reactivity manipulations.

The following control manipulations and plant evolutions are acceptable for meeting the required reactivity control manipulations. The starred ( *)

items shall be performed on an annual basis t the remaining items shall be performed in a two-year cycle. Those control manipulations that are not performed at the plant may be performed on a simulator. Credit for these activities will be limited to one or two operators performing the task and one or two senior operators engaged in directing the operations.

  • - Loss of all feedwater (normal and emergency

- Loss of service water and emergency service water

- Loss of shutdown cooling Loss of component cooling system or cooling to an individual component

3. Des [qn,, Procedure, and License Change Review This review basically consists of a periodic (i.e. ,

mon thly) required reading information packet for all reactor and senior reactor operators, STA's, and SRO-Certified Instructors at the facility, and ensures that licensed operators and senior licensed operators review revisions to the Operating Technical Speci fications and Environmental Technical Speci f ica tions, review significant procedure changes, review completed facility design changes, plant expe riences, industry events, and NRC/ Utility Reports or publications which are determined to af fect plant opera t ions.

When Ehe Plant Operations Review Committee, th e Station Manager, Station Superintendents, Operations Engineer, Technical Engineer, or the Training Coordinator determines that a procedure revision or completed design change af fects plant operations, it is included in this review process. Presently, th ere exists a reading list which is distributed on a nornnt monthly basis and includes those items (as mentioned above) which should be read and reviewed by personnel who possess an RO or SRO license. This group is designated by the Station Manager and maintained by the Training Coordinator. This mandatory reading material is signed on a receipt acknowledgement form (Figure 2) and returned to the Training Coordinator to be maintained. These sheets shall be returned for record purposes no later than 60 days from date of i s su e . This time frame of 60 days is considered accep table for three (3) reasons:

(1) The work schedule of a typical RO/SRO/STA individual will allow the LOR participant one and in some cases, two weeks of Utility Shif t to read the multitude of materlat considered applicable to a licensed operator.

Licenced Operator Rrqualification Training Program Page 7 of 14 (2) If immediate attention (i.e. , within several days) notice is needed for licensed operators, then the Station Staf f (normally the Operations or Technical Engineer) will have placed in the Shif t Notebook or via Night Orders the applicable priority material. All on-shi ft licensed personnel will normally have this discussed in their next shif t turnover meeting. This ensures rapid dissemination of critical material.

(3) These mandatory reading information packages are to be added to the classroom lecture series of the DOR program every 6 weeks to ensure that this impor tan t information is highlighted in a separate training atmosphere away from reading material on-shift. This will ensure all timely /important information is synopsized and discussed again with the operator af ter he/she reads it in the required reading package.

The administrative mechanisms for this program will take into account changes which would require immediate operator notification. As mentioned, notification of all operators may occur by an appropriate shi f t meeting discussion, or a shi f t note from the Operating Engineer or alternate. Changes within the scope of this program may also be reviewed Ehrough lectures or other verbal presentation by appropriate personnel. In such cases, records of participation shall be developed and retained.

4. Procedure Review In order to ensure a continuing awareness of the action and responses necessary during abnormal and emergency situations, each licensed operator and senior licensed operator will review the content of all Emergency (E) Procedures, all Operational Trans ient ( OT) Procedures, Of f-Normal (ON) Procedures, Transient Response Implementation Plan (TRIP)

Procedures, and designated Security and Emergency Plan Implementing Procedures annually.

Such reviews will be accomplished in one or more of the following ways

- Actual performance of the procedure under abnormal or emergency conditions Drills utilizing a simulator such as the Limerick Training Center Simulator

'Page 8 of 14 A simulated wal%-through of the procedural steps necessary to cope with the situation

- Lectures or discussions conducted by shif t supervision or other appropriate personnel-(as documented by LOR lecture series participation or Shif t Meeting Notebook /Shi f t Night Orders.

- Self-study of the procedure (as added to required reading material)

B. Evaluation of Performance The following methods of evaluating the continued satisfactory performance of an LOR participant are utilized during the LOR program yearly (1) Annual Written Exams Annual written examinations shall be given to provide a basis for determining operator knowledge for all RO/SRO/STA/SRO-Certified Instructor personnel and also to assist in determining areas in which retraining is needed. The examinations will be operationally oriented and will be comparable in both scope and degree of dif ficulty to the NRC license' examinations for the applicable license held. Exams will be administered annually (every 12 months) for all LOR par ticipant s. In most yearly requal periods, exams shall be given twice a year to allow examining of 50%

of the LO/ SLO /STA/ Instructor personnel every 6 months.

This minimizes the number of LOR participants Who take the e.nnual exams at any one time. As discussed previously, operators scoring above 80% in all sections of the annual written exam will not be required to attend requalification lectures unless attendance is required for all licensees. An operator Who is evaluated at less than 80% in a section will be required to participate in requalification lectures in that section. An operator Who is evaluated at less than 80% on the overall examination or less than 70%

in any section, will be placed in an accelerated review program upon concurrence from the LGS Station Manager and Senior Plant Staf f. In addition, an operator Who is evaluated at less than 80% on the overall examination or less than 70% in any section will be immediately scheduled for an oral examination during the same week in which the written exam is administered in order to determine the necessity for removing that operator from his licensed shif t duties during this accelerated review. The words "immediately scheduled" speci fically address Ehe I

Licensed Operator Requalification Training Program Page 9 of 14 intention to administer an oral examination (given by Plant Staf f and Training Staf f) within the same week that the written exam is taken and graded. This ensures immediate feedback on whether or not the licensed operator should maintain his licensed duties.

It should be noted tha t in the interval between the operator taking the annual written exam and being administered a plant staf f oral exam, that individual is in a training mode and will not normally be a member of the shif t exercising his licensed duties.

This ensures tha t the operator is ef fectively removed from licensed duties until determination by oral examination is made by plant and training staf f.

Written re-evaluations will be made following the review program until satisfactory performance is demonstrated by scoring 80% or greater on the overall written test and/or 70% or greater in each section.

The operator will also participate in all applicable subsequent requalification lectures for next year's period.

(2) Annual Plant Oral Examinations Each operator and senior operator, STA and SRO-Certified Instructor shall participate in an annual oral examination with a designated station staf f or qualified training staf f member. This is accomplished by a walk-through examination which includes oral examination on aspects such as limitations, setpoints, trips, alarms, operator response, phys ical characteristics of the plant, and methods for placing equipment into operation. This type of examination (which is patterned af ter an NRC oral exam) provides systematic observation and evaluation of th e performance and competence of all license holders and LOR participants. Ope ra tor s, STA's, or instructors whose overall performance in the Annual Oral Examination is unsatisfactory, will he placed in a training review program to correct the deficiencies noted. He will then be given another Oral Examination to ensure proper retention of material discussed in the review program and provide evidence that his knowledge level is satisfactory to continue his licensed duties.

(3) Annual LGS Simulator Demonstration Examinations Each licensed operator, STA, and SRO-Certi fied ~

Instructor shall participate in an annual simulator demonstration examination utilizing the plant-re ferenced control room simulator in the Limerick Training Center. This examination is utilized to

Licensed Operator Requalification Training Program Page 10 of 14 evaluate the operational skills, transient response (i.e., abnormal and emergency operations), and control manipulation performance of each LOR participant in the discipline in which he is qualified. Again, this simulator demonstration examination is similar to that administered by the NRC. Shi f t Technical Advisors (STA's') are evaluated on the simulator in the job per formance activities relating to an on-shi f t STA.

This simulator evaluation incorporates the systematic evaluation of the performance and competency of licensed operators and STA's by supervisors and/or qualified training staf f members including evaluation of that operator's actions taken or to be taken during actual or simulated abnormal and emergency conditions (as stated in 10CFR55, Appendix A, Part 4.c).

No r mally, qualified training staf f members will evaluate the performance of the individual licensed operators or STA's being examined on the simulator.

(4) LOR Cycle Weekly Subject Matter Quizzes Written quizzes will be administered at the conclusion of each lecture series session. For those who attendance was mandatory at the lecture, a grade less than 80% will indicate a need for additional training in that subject. The supplementary training will be under the supervision of an instructor designated by the Training Coordinator, Shi f t Supervision, or sel f s tudy by the individual involved. A subsequent quiz must be taken and grade of 80% received to allow satis factory credit Sor completing this weekly LOR training.

(5) Control Room Hours Licensed staf f members will have a minimum of 16 hours1.851852e-4 days <br />0.00444 hours <br />2.645503e-5 weeks <br />6.088e-6 months <br /> of participation in control room operations over a four month period. This participation should include a senior supervisory function or actual reactor operations as an assigned or extra person on the shift. " Participation in control room operations" is defined as those control room functions which an RO or SRO LOR par ticipant per forms, observes, or supervinos and which has direct applicability toward maintaining his on-shift level of knowledge as a licensed RO or SRO.

LicGnsed Oparator R qualification Training Program Page 11 of 14 Coordinator or designee to demonstrate that his knowledge and understanding of the facility are sati s factory. The Training Coordinator in conjunction with the plant staf f will then inform the NRC that said individual is certified to perform licensed duties. This is required as per 10CFR55.31(e) .

(6) LGS Senior Plant Staf f certification Board This is an oral (examination) boari given to all LO and SLO personnel normally just prior to Eheir license renewal with th e NRC. The purpose of this oral board is to assure / certify to the V.P.-Electric Production that the licensed operator is capable of discharging his responsibilities competently and safely. This cert. board consists of a member of the Senior Plant Staf f, an operations or technical group (SRO licensed individual) member, and a qualified member of Ehe LGS Training Staff. The licensed operator must pass an oral cert. board prior to being recommended for license renewat by PECo management.

(7) Walvers or Exemptions from Taking Annual Written, S f' m u la t or , or dral LOR Examinations Authors, reviewers, or graders of LOR requalification exams will be exempt from taking those annual examinations with which licensee or training staf f participation has been utilized by the Training Coordinator in exam preparation. Normally, the Training Coordinator will select a total group of 2-4 members from Ehe Senior Plant and Training Staf fs to assist in written and/or simulator exam preparation, review, and/or grading. These personnel will be rotated from year to year to ensure that the same license or certification holders will not be continually exempt from taking annual examinations.

For example, one year might f ind the Operations Engineer and Station Superintendent-Operations plus the LGS Training Coordinator and one SRO-Certified Instructor writing, reviewing and/or grading the annual written examination. Those people wouli Ehen be exempt from the annunt written only. They would still be required (as license or certification holders) to participate in the other phases of the annuni evaluation process (i.e., oral, simulator, cert. board) if they were not designated by the Training Coordinator to assist in the preparation and administration of those other exams. All LGS Senior Staf f and Training Staf f eligible personnel will be used on a rotating basis from one year to the next.

C. Records Each licensed operator and senior operator will keep current in the designated record book his record of plant reactivity changes. (See Figure 1) This record is combined with the operator's record of per formed reactivity manipulations in the plant referenced LGS simulator to ensure that the required manipulations are completed each year as mentioned in Sect. A.2.

The Shift Superintendent and LGS Training Staf f shall be responsible to see that an individual's participation in a shi f t training session is recorded.

Records shall be maintained of the Requalification Program to docunent each licensed operator's and senior licensed operator's participation. The records shall contain copies of individual annual examinations and lecture quizzes administered; and results of evaluation and documentation of additional training.

Af ter an NRC-NRR audit, copies of audited individual examination answers may be destroyed if Ehey meet the two (2) year retention criteria as specified in 10CFR55, Appendix A, Part 5.a.

D. Quality Assurance The lesson plans, in s truct ions, procedures and drawings used in the requalification lecture training programs will be approved by the Training Coordinator or designee.

Assurance of technical adequacy and accuracy of the training is the joint responsibility of the Technical Engineer and/or Operations Engineer as well as the Training Coordinator.

Audits of Ehe requalification program are made by Philadelphia Electric Company Quality Assurance Division.
