APPENDIX A NOTICE Of VIOLATION Arkansas Power & Light Company Docket:

50-368/86-13 Arkansi.s Nuclear One, Unit 2 License:

NPF-6 During an NRC inspection conducted during the period April 1-30, 1986, a violation of the NRC requirements was identified. The violation involved use of an inadequate procedure for maintenance on the 'C' high pressure safety injection pump.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:

Technical Specification 6.8.1 requires that procedures for performing maintenance shall be established, implemented, and maintained; and 10 CFR 50, Appendix B, Criterion V, requires that, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."

Contrary to the the above, Procedure 2402.36, "2P89 High Pressure Safety Injection Pump Maintenance," Revision 2, was used for the replacement of the stuffing box extension gasket on the outboard end of the

'C' high spressure safety injection pump in April 1986.

This procedure contained deficicncietmaking it inadequate to properly control the job; however, the job was succe'ssfufly completed due to the experience and training of

the craf tsman compensating for the procedural inadequacies.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Arkansas Power & Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
