BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - FAILURE TO PERFORM ANALYSIS CHECKS ON ISOMETRICS AND LOAD TABLES - BLRD-50-438/82-75, BLRD-50-439/82 FOURTH INTERIM REPORT The subject deficiency was initially reported to NRC-OIE Inspector R. V. Celenjak on October 26, 1982 in accordance with 10 CFR 50.55(e) as NCR BLN CEB 8215. This was followed by our interim reports dated November 24, 1982, March 21, 1983 and March 28, 1985.

Enclosed is our fourth interim report. We expect to submit our final report no later than six months before fuel load.

FOURTH INTERIM REPORT Description of Deficiency Problems N4-1KE-F, N4-2KE-F, N4-1NK-B, and N4-2NK-B isometrics and load tables were signed out and issued befbre the analysis check was performed.

On problems N4-1KE-F and N4-2KE-F, the analysis check has now been performed and several analysis errors have been found which are significant. The cited isometrics and load tables are used in the design of essential c:w cooling water (ERCW) pipe supports and reactor coolant drains and vents (NK) cystem supports.

The cause of this deficiency can be attributed to a lack of procedural controls on checking activities.

Interim Progress TVA has completed the review of most of the problems within the scope of this de ficiency. Some support redesign has been required as a result of our review. However, TVA has not reviewed problems N4-1NK-E and N4-2NK-E, which were included in the scop.e of this nonconformance in our third report.

The rigorous analysis handbook has been updated to include a policy statement (section X, policy statement 21) which clarifies the items requiring completion before management review and endorsement.

TVA will provide a final report documenting our review of these problems upon completion but no later than six months before fuel load.
