APPENDIX A NOTICE OF VIOLATION Houston Lighting & Power Company Dockets: 50-498 South Texas Project, Units 1 and 2 50-499

. Permits: CPPR-128 CPPR-129 During an NRC inspection conducted on February 10 through March 13, 1986, one violation of NRC requirements was identified. The violation involved failure to properly inspect. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:

Failure to Properly Inspect Criterion X of Appendix B to 10 CFR Part 50 requires that activities affecting quality be subjected to an inspection program to verify conformance with th documented instructions, procedures and drawings for accomplishing the activ'4y. This requirement is amplified by the approved Quality Assurance Plan Description (QAPD) for the South Texas Project.

QCP 10.15, Revision 3, paragraph 5.2.3.8 states that the Quality Control Inspector (QCI) shall verify all field welds required have been inspected and accepted by recording the completed acceptable welding inspection report number on the installation inspection package.

Contrary to the above, the weld configuration on Panel 3E241EDP001 differed from the weld configuration shown on "as built" drawing 14926-4362, indicating that the panel installation had not been properly inspected to verify that the field welds were in accordance with instructions.

This is a Severity Level IV violation (Supplement IE) (498/8604-01).

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting & Power Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
