((. ) STEAM GENERATOR TUBE EXAMINATIONS 1.0 Introduction This Sununary Report describes llouston Lighting & Power Company's (HL&P's)Insenice inspection (ISI) of the Steam Generator (S/G) tubing at South Texas Project Electric Generating Station, Unit 1 (STPEGS-1) during the 1REO7 refueline, outage.

The steam generator tube 151 is required by Sections 3/4.4.5 and 4.0.5 of the Technical Specifications to be performed in accordance with the edition and addenda of the American Society of Mechanical Engineers ( ASME)Section XI Code "Insenice inspection of Nuclear Power Plant Components" prescribed by Title 10 of the Code of Federal Regulations, Part 50, Section 50.55a(g). Therefore, the ISI is required to meet the 1983 Edition of the ASME Code Section XI with addenda through the Summer 1983. The first ten year inspection intenal of STPEGS-1 began August 25,1988. Because STPEGS-1 was out of senice continuously for 13 mcnths, the inspection interval was extended for an equivalent period in accordance with IWA-2400(c) of the ASME Code Section XI. This extended the inspection interval to September 24, 1999. The ISI summarized herein constitutes the seventh ISI performed dur.ng the first ten year inspection interval. d The STPEGS-1 ISI program for the first inspection interval is described in the Ten Year ISI Plan previously filed with the Nuclear Regulatory Commission (NRC) and the State Of Texas. The STPEGS-1 program was developed and is being implemented in accordance with 10CFR50.55a, IWB (Examinatica Category B-0) of the 1983 Edition of Section XI Code with the Summer 1983 Addenda, HTPEGS Technicel Specification 4.4.5, and other regulatory and Code cases as specified in the 'I en Year ISI Plan.

This Summary Report satisfies the reporting requirements of ASME Code Section XI, IWA-6000 and 4.4.5.5 (b) of the STPEGS Technical Specification.

Section 3/4.4.5 of the Technical Specifications also provide alternate repair criteria for outer diameter stress corrosion cracking at tube support plates and primary water stress corrosion cracking of tubes within the tubesheet. This report discusses aspects of the implementation of alternate repair criteria. A separate report discusses the implementation of the altemate repair criteria for outer diameter stress corrosion cracking at tube support plates in detail and responds to the requirements of BAW-10204P "Scuth Texas Project - Unit 1 Tube Repair Criteria for ODSCC at Tubes Support Plates", paragraph 13.3.

The STPEGS-1 plant design conta".ns four (4) Westinghouse recirculating design model E2 steam generators. Each model E2 steam generator was designed and fabricated by Westinghouse Ek ctric Corporation of Tampa, Florida. Each steam generator contains 4864 tubes. The tubing p materia! is low temperature mir mnealed ASTM SB-163 Inconel having a nominal outer diameter Q (OD) of 0.750 inch and nonnu wall thickness of 0.043 inch. The nominal thickness of the tubesheet is 22.65 inches.

g 2.0 Scope of Examinations

The STPEGS 1 Ten Year ISI Plan describes the ISI program for examination of steam generator tubing. Additionally, an auxiliary ISI plan (ISI Outage Plan) entitled, "1997 Outage Plan for the In-senice inspection of Steam Generator Tubing at the South Texas Project Electric Generating Station Unit 1" was prepared by WNSD and approved by HL&P. The ISI Outage Plan identified the steam generator tube areas expected to be examined by eddy current testing (ET) and the ET procedures expected to be used during the ISI.

The initial scope consisted of the following planned examinations:

Bobbin coil ET of all in service tubing over its full length, in each steam generator. In addition to the evaluation for tubing degradation, a single auto-analysis was to be performed for an evaluation of the following: (1) dents and dings equal to or greater than 3.0 volts, (2) tube support plate ligament condition, (3) sludge heights in each steam generator, and (4) interfering signals, e.g., mixed residuals, copper signals, etc., that could cause misinterpretation of an ODSCC signal at a tube support plate intersection.

Motorized Rotating Pancake Coil (MRPC) ET of the hot leg top of the tubesheet (TSH) for each insenice tube in each steam generator. The area ofinterest to be examined extended one (1) inch above to two (2) inches below the secondary face of the tubesheet.

/^S U MRPC ET concurrently of a first, second and third sample of tubes at the top of tubesheet intersection in the cold leg (TSC) of each steam generator. The area ofinterest to be exa. , ced extended one (1) inch above to two (2) inches telow the secondary face of the tubesheet. These samples conipused areas in more than twenty-two (22) percent of the in-senice tubes.

Plus point coil ET of the U-bend section of all tubes in Row 2 and twenty (20) percent of the tubes in each of the Rows 1 and 3 in each steam generator. The area to be examined extended from the uppermost tube support plate on the cold leg side to the uppennost tube support o.' the hot leg side of the bundle.

MRPC of 257 Special Interest exammation areas in 198 tubes, including the specialhhg program discussed below. These Spec'al Interest examination areas consist of top of thu tubesheets, tube support plates, baffle plaies in the pre-heater region, and free-span regions where previous indications existed or where experience indicated a higher potential for degradation. During the course of the outage, a total of 6 indications in 5 tubes were removed from the program, by Outage Plan Change Form, based on fmdings during the 1RE07 inspection and baseline review process.

Included in the above SpecialInterest program was an exae/ nation of a 20% random

,, sample of all freespan dings equal to or greater than 5 Volts. The sample was chosen to cover tne 20% quantity (62) ofdings but by picking tubes (where possible) which had (3-) multiple dings meeting the criteria in an effort to control inspection time. In addition, if there were any other dings in a tube selected, even if below the 5.0 Volt sort criteria, they 3

-Q were included in the population to be tested. This program resu_hed in 95 examinations in 54 tubes and included 64 ding indications greater than 5.0 volts. All of these inspections ~

. would extend over an approximate length of four (4) inches, as a minimum.

This scope exceeded the requirements for the first ISI sample of tubes specified in 4.4.5.2 of the STPEGS technical specification (NUREG - 1346). The ISI Outage Plan is consistent with the'

requirenwnts of the Ten Year ISI Plan.

4 The personne.1 who performed the ET acquisition and analysis during the 1REO7 outage were employed by WNSD, NDE Technology, Anatech International, Quantum NDE Services, Core iStar intemational, Laboratory Commercial Senices, Northem States Power, Korea Power ;

Systems,- Inetec,' Zetec, and Framatome Technology, Inc. (FTI). They were certified in -

accordance with the requirements ofIWA-2300 of ASME Code Section XI and the certification practices of their respective ernployers. Ninety4ix (%) percent of the production ET analysts _ i were certified as Qualified Data Analysts (QDA's) for the steam generator work performed. It is noted that the growth rate evaluation for Ahernate Repair Criteria (ARC) data was performed by

. one QDA and four non-OA's. A QDA is a LevelIIA or III who has passed rigorous testing of -

, his ability to analyze a random selection of expertly-judged indications, from the Electric _ Power

' Research Institute's "PWR Steam Generator Examir.ation Guidelines", Appendix G Performance Demonstration Database for various steam generator designs and all types ofknown defects.

. Before an ET data analyst could perform any analysis during iREO7, they were required to

! successfully complete a STPEGS plant site specific steam generator ET data analysis training program. The analyst training program consisted of(l) ET data analysis course and (2) site o specific laboratory / practical training. The ET data analysis course consisted of a classroom 2 lecture (or video tape) and reading of a training manual and the analysis procedures. The course addressed the specific STPEGS steam generator design, operating history, presions ET resuhs,

' the data acquishion procedure, and analysis procedures to be used for the STPEGS Unit I ET examinations. The site specific laboratory / practical training included hands-on review of the flaw
indications of the types that have been identified at STPEGS or at other plants of simdar design to STPEGS. Successful completion of the site specific course required the passing of a written and g - a practical (hands on) test, which could be limited to either bobbin coil, MRPC or both, depending on staffing requirements.

For personnel involved in the performance of the ET acquisition anNor data analysis, their certification levels and identification numbers are listed in Appendix A. The identification

numbers of the personnel that acquired or analyzed the ET data for a specific examination are recorded on the optical disks and data sheets. The identification number consists of the first letter L of the persons last name and the last four digits of their social security (or passport) number.

' .i.2 Examination and Analysis Procedures

! All ET inspections were performed in accordance with tlw WNSD procedure MRS 2._4.2 GEN-35, Revision 6 entitled, " Eddy Current Inspection of Presenice and Insenice Heat E* hanger Tubing" with Field Change Request No.1. This procedure, with the change authorization, is ,

demonstrated to the satisfaction of the Authorized Nuclear Insenice Inspection (ANII).

i i@adative. The faster probe pull speed was demonstrated to the satisfaction of the ANII in

, - acco da 1 with IWA-2240 of Section XI. An additional ACT Sheet, TGX-01 A, was used for '

thet 3 of the U-bends of the Rows 1,2 and 3 tubes. This permitted a slower no ninal pull speed *nches per second with a digital signal sample rate of 850 samples per second or greater to reduce the effects ofprobe snap through the U-bend.

The MRPC ET examination techniques were performed using the examination frequencies of 300, 200,100, and 10 kHz in the absolute nxxle. All combinations of possible MRPC probe configurations and/or locations, including contingencies, that could have been encountered are covered on ACT Sheets TGX-02/02A, TGX-03/03A, TGX-04, TGX-05 and TGX-06.

. The ET data analysts performed _ the analysis per Westinghouse procedure NSD-FP-1997-8053

Rev. O, titled, " Steam Generator Eddy Current Data Analysis Procedures", STPEGS Document

No. 0120-09-00077-AWN. All bobbin coil and MRPC ET data for each steam generator tube O were subjected to two (2) separate independent analyses in accordance with HL&P Engineering Instruction El-8.02, entitled " Steam Generator Eddy Current Data Control", Revision 1, except that analysis for dents, dings, support structure degradation and sludge depth was limited to a single analysis.

WNSD and it's subcontractors performed the primary analysis in accordance with STPEGS document no. 0120-09-000036-AWN. FTI and it's subcontractors performed the secondary analysis in accordance with the FTI procedure 54-ISI-400-5 entitled " Multi-frequency Eddy Current Examination of Tubing" with Safety Document Change Notice No. 30-5000588. This FTI Procedure, with its change, is STPEGS document No. ST401280-00231-A2U.

The primary data evaluation for degradation was performed manually by the analysts using the Westinghouse ANSER software. Ni single analysis for dentsMings, support structure degradation and sludge depth was performed using ANSER auto-analysis routines. The

_ secondary data evaluation was performed manually using the Zetec Inc. Eddynet 95 software.

~

The ET primary and secondarv data analysis was performed in separate facilities. The primary analysts were located at the WNSD Remote Eddy Current Data Analysis Center (REDAC) in

' Madison, Pac The secondary analysts were located at the Rockridge facilities in Benicia, Ca.

Results of all of the eddy current examinations were recorded on a digital rewritable optical disk and the final resolutim data' sheets, which are stored as records. Each disk contains the raw ET 1 i 7p data, secondary, and resolution resuhs for each calibration group. The optical disks also contain

G! - the system calibration and calibration verification with the dates and times for each calibration and verification. The unique number of the digital rewritable optical disk and the cahbration group
number have been recorded on the data sheets and optical disksi Therefore, the system 6_

- and WNSD ANSER 8.1, Rev 75 soRware. The cokilegs were tested utilizing the Zetec MIZ-4 30A test instruraent and Zetec Eddynet '95 version 4.0 Patch 14 soRware.~ The primary analysis was performed using ANSER soRware, ANSER 8.2 Rev 30. This soRware is produced by ;

WNSD. The secondary analysis was performed using Eddynet '95 Version 4.0 Patch 14 soRware.

Both test instruments and soRware store and process the ET data in a digital format and have a -

mignihntly improved dynamic range and signal-to-noise ratio as compared to analog systems.

Both test systems are capable of being opereted at tocations remote from the steam generators

. (e.g., in low radiation areas).

The ASME Boiler and P1 essure Vessel Code, Nuclear Components Code Case N-401-1, " Eddy Current Examination-Section XI, Division 1", was used aRet the digital equipment and _the ET technique used were demonstrated to the STPEGS ANII in accordance with Case N-401-1.

U-bend acetions of tubes in rows 1,2 and 3 were examined from either leg depending on schedule considerations using either a 0.590 or a 0.610 inch diameter magnetic-biased bobbin coil ET 1f probe. The test extent was from the top support plate on the far side (side opposite from which the probe entered the tube) to at least the second support plate from the top on the near side. The straight lengths in both legs of these low row tubes were tested separately with a 0.610 inch

, magnetic-biased diameter probe. For all other tubes, the bobbin ET examinations wen performed p with a 0.610 inch diameter magnetic-biased probe.

l- In conducting MRPC ET in the straight sections of tubing, at TSP's for those bobbin coil indications related to Ahernate Repair Criteria and during the TSHffSC programs, WNSD used 0.610 inch diameter three (3) coil MRPC probes. These MRPC probes contained one 0.115 inch diameter pancake coil, one coil sensitive to axially oriented flaws and one coil sensitive to circumferentially criented flaws.

All circumferentially oriented and mixed-mode indications found durmg the TSH MRPC program and those bobbin coil TSP ligament indications were subjected to inspection with a 0.610 inch diameter plus-point RPC probe. This 3-coil probe design consists ofone (1) phu-point coil y' - (interwoven axialty and circumferentially oriented coils that are differentially connected), one (1)

O.115 inch diameter pancake coil and one (1) shielded high frequency 0.080 inch diameter pancake coil.

The low row ubend RPC program was conducted with a 0.580 inch diameter single coil plus-point probe. This probe was also used inspect a freespan non-quantifiable indication situated very

- near the cold leg U-bend tangent point in R43C46 in C steam generator.

}3.4 ' Calibration Standards -

The calibration standard design used for bobbin coil inspections was a combination in-line :

, standard. It contained ASME Code Section XI flaws, drill holes for measuring probe wear, simulated AVB wear flaws, a small derc, and both carbon steel and stainless steel support rings.

The calibration standard utilized for the great majority of the hot leg MRPC wm a guide tube design containing several ID and OD axial and circumferenthi electro discharge inachined (EDM)

- notches in sufficient quantities to pennit the sizing ofindications if n-cessary. All MRPC ET conducted at the locations of bobbin coil ET distorted support indications was performed using

" standards oithis design. The ET calibration standards used for the cold leg MRPC inspections .  !

. were also a guide tube design but incorporated both flat bottom holes and only two (2) EDM -

notches. - This design was also used for a small number of hot leg examinations. The number of EDM notches in the cold leg standard design were not adequate to allow sizing, thus for those o inspections that required sizing it was necessary to utilize a separate hand-held SDM notch sizing standard. Also, a baffle plate wear scar standard was used to size indications found at cold leg l baffle plates.

The standards used to inspect the U-bend regions in Rows 1 and 2 were fabricated from UNS Alloy N06600 of the same material specification and same nominal size as that of the tubes examined in the steam generators. The U-bmd areas of the Row I ami 2 tubes in each steam .

generator have received an in-situ hes' treatment to improve their resistance to stress corrosion I\ cracking. The ET cahhation s:.indards used were not subjected to that heat treatment; however, -

4 k)- L 4.0 - Summary ofExaminations Bobbin coil ET techniques were performed on one hundred (100) percent of all in-service tubes in each of Steam Generators A, B, C, and D. All of these tubes were _ examined over their full length

(from tube end to tube end). The numbers of tubes which were examined by the bobbin coil ET technique are as follows

and dings equal to or greater than 3.0 volts, (2) tube support plate ligament condition, (3) sludge heights in each steam generator, and (4) interfering signals, e.g., mixed residuals, copper signals, etc., that could cause misinterpretation of an ODSCC signal at a tube support plate intersection.

MRPC ET examinations _were conducted at 100% of the hot leg top of the tubesheet (TSH) intersections. All single circumferential indications (SCI) and multiple circumferential indications

.O (MCI) detected during the TSH exammation were re-examined with a plus-point coil to permit detailed crack characterization in accordance with the document "EPRI Alternate Repair Limit for-Circumferential Cracking at Expansion Transitions: ECT, Data Analysis, and Engineering Data Requirements". In addition to the TSH MRPC program. .u ample MRPC progr. m was also i undertaken at the cold leg top of tubesheet (TSC) interfact. This sample program contained 1086 tubes (approximately 22% of the tubes) in each steam generator. There were no findings that necessitated expansion of the TSC MRPC program.

MRPC ET was also conducted at the various tube locations other than at TSH/fSC, as listed in Appendix B. The listings of Appendix B includes, in part, all locations where bobbin coil ET a~ indicated a signal at a tube support plate (TSP) which may be (1) possible ODSCC exceeding 1.0 volt,-(2) possible ODSCC in the Alternate Repair Criteria TSP Plugging Limit Exclusion Zone, (3) a dent exceeding 5.0 voks, (4) indicative of support plate ligament degradation, or (5) an

. interfering signal, e.g., copper or mixed residual of sufScient magnitude to 'cause a 1.0 volt

ODSCC indication to be_ missed or misread.- In addition, Appendix B includes all freespan (both
abov- .xl within the tubesheet) non-quantifiable ET indications, e.g., NQI, DNI, DTI, etc., which cc la not be verified to be present in baseline data, that were examined by MRPC for final disposition. Further, selected freespan dings that were inspected to insure that there was no cracking being masked by the dings are also listed in Appendix B.

4 5.0 : Examination Resuhs and Co Tective Actions

- The location of the indications were recorded relative to the adjacent tube support, baffle platef and/or anti-vibration bar. The tube support plates and baffle plates were numbered consecutively  :

from 01 H to 10H (on the hot leg) and from 11C to 23C (on the coki leg) starting on the hot leg side, over the U-bendi and down the cold leg side of the steam generator. The antinibration bars

were numbered AVI, AV2, AV3, and AV4 from the hot leg to the cold leg side, respectively.

. Indications in the tubesheet. area were recorded relative to TEH or TSH (hot leg) or TEC or TSC. r

. (cold leg) depending on whether the indications were at the tube end (E) or secondary face (S). .

-In addition, the vertical distances from these landmarks to flaws were recorded, with positive being upward from the reporting structure. In the U-bend region, the positive direction was .

measured from one land mark to the next, progressing from the hot leg to the cold leg, i.e., an

. indication between AVI and AV2 was located as AV1 + x.xx inches.

- ET indications contained in this report were assigned three letter codes. The indication codes ' ,

reflect the susnected nature of the discontinuity, Some of the codes used are as follows:

- DNG - ding

. DNI - ding or dent containing a possible indication DNT - dent DSI - distorted support with possible' flaw indication .

MAI- muhiple axialindication MBM - manufacturing burnishing marks MCI - multiple circumferential indications MVI - multiple volumetric indication NQI - non-quantifiable indication SAI- single axialindication SCI - single circumferential indication SVI - single volumetric indication Lists, including the locations and depths, of all flaw indications which were characterized as

- reductions in the tube wall thickness are contained in Appendix C. Also included in Appendix C are the SAI, SCI, MAI and MCI indications detected by MRPC ET. SAI, SCI, MAI and MCI 1 indications are expected to be the resuk ofcracks. These indication classifications may be considered to be reductions in the tube wall thickness. All of these indications were plugged regardless ofdepth.

In August 1997, a review ofpreviously called free span indications was performed to' establish tracibility of these already identified signals to the baseline bobbin coil data. In addition, during the 1RE07 outage, any free span ~ call which did not have a corresponding record of a baseline call was also subjected to a review of the baseline data. The purpose of this effort was to let those

. Ltubes with corresponding signals in the baseline, which had not changed, to remain in senice.

. The great majority of these imperfections, when tested by MRPC, were shown to be volumetric.

Those MRPC volumetric indications which could not be found in the baseline data remained as SVI/MVI and were ultimately removed from senice. These tubes are also listed in Appendix C.

No copper deposits, circumferential cracks at tube support plates, or axial cracks extending beyond the edge of the tube support plates were detected by either bobbin coil or MRPC testing.

Further, from the bobbin coil data, there was m reportable evidence of sludge height in any of the steam generators on either the hot or cold leg secondary face of the tubesheet.

Based on fabrication records, it was known that Row 1 tubes in C and D steam generators at tube sheet support plates 19C and 21C and Row 48 in steam generator C at elevations tube sheet support plates 18C and 20C had been trunmed into adjacent tube holes during manufacture and may very well be identj6ed as Possible Support Indications (PSI) during the bobbin coil data evaluation of the condition of tube e pport plate . Exclusive of those expected, only nineteen (19) were called and only seven (7) of those were deemed candidates for fhrther RPC inspection with a plus-point coil. The plus-point coil inspection of those seven (7) candidates revealed no evidence of service induced degradation, e.g., cracking or erosion, in the support plate ligaments.

Lists of all dents and dings detected in each steam generator are included in Appendix D.

The total numbers of degraded and defective tubes detected during the bobbin coil ET and MRPC ET are as follows:

5.1 Steam Generator A Thirty-five (35) areas of tube degradation, as dermed by the Technical Specifications in 4.4.5.4, f]

U were detected in thirty-two (32) tubes. Of these tubes with indications of degradation, thirty-one (31) were defective as dermed by the Technical Specification 4.4.5.4 Steam Generator A had seven (7) areas of degradation detected by bobbin coil /MRPC at locations other than in the TSIVfSC transitions in seven (7) tubes. Of these tubes with indications of degradation detected by bobbin coil /MRPC at locations other than the TSHfrSC transitions, six (6) were considered to be defective. The SAI at 03H, associated with a DSI less than 1.0 Voh, in tube R28C36 is included in Appendix C, but it is not counted as degraded or defective because of tube support plate Alternate Repair Criteria.

Steam Generator A had twenty-three (23) tubes with twenty-four (24) areas of degradation detected by MRPC in the TSH transition. Of the tubes with degradation, all twenty-three (23) were considered defective.

Three (3) tubes contained volumetric degradation in the TSC MRPC program. Of tnese, tube R2C83 also contained volumetric degradation detected by MRPC at TSH. No supporting baseline infonnation could be found to support leaving these tubes in-sersice.

Five (5) tubes were preventatively plugged, three (3) of which surrounded two (2) tubes that exhibited possible loose parts (PLP's).

No areas of degradation were detected by rotating probe ET in the U-bends of rows 1,2 or 3 tubes.

t 5.2 ' Steam Generator B ?

Thirty four (34) areas of tube degradation, as defined by the Technical Speci6 cations in 4.4.5.4, were detected in twenty-eight (28) tubes. Of these tubes with degradation, all twenty-eight (28) .

tubes were defective as defined by the Technical Specification 4.4.5.4.

Steam Generator B had one (1) area of degradation detected by bobbin coil /MRPC at locations other than in the TSHfrSC transitions in one (1) tube. This one (1) indication of degradation detected by bobbin coil /MRPC at a location other than the TSH/TSC transitions was considered to be defective.

Steam Generator B had thirty-three (33) areas of degradation in twenty-seven (27) tubes that

, were detected by MRPC in the TSH transition. Of the tubes with areas of degradation, all twenty-seven (27) were considered to be defective.

No areas of degradation were detected by rotating probe ET in the U-bends of rows 1,2 or 3 tubes nor was there any degradation detected in the TSC MRPC program.

' 5.3 ' Steam Generator C Forty-nine (49) areas of tube degradation, as defined by the Technical Specifications in 4.4.5.4, were detected in forty-four (44) tubes. Of the tubes with indications of degradation, all forty-four N (44) tubes were considered to be defective as defined by the Technical Specification 4.4.5.4.

These numbers include two (2) tubes exhibiting the presence of both a loose part and significant wear. In addition, tube R360107 was obstructed to a 0.560 inch diameter probe and was plugged.

Steam Generator C had twenty (20) areas ofdegradation detected by bobbin coil /MRPC at Deations other than in the TSHfrSC transitions in sixteen (16) tubes. Of these tubes with indications of degradation detected by bobbin coil /MRPC at locations other than the TSH/TSC transitions, all sixteen (16) were considered to be defective.

Steam Generator C had twenty-eight (28) areas of degradation in twenty-seven (27) tubes which were detected by MRPC in the TSH transition. Of these twenty-seven (27) tubes with

, degradation, all were considered to be defective.

In the TSC MRPC program, there was one (1) tube exhibiting a volumetric indication for which there was no supporting baseline data to allow this tube to remain in senice.

The minimum required size of the initial ISI sample is defined in Technical Specifwation 3/4.4.5 Table 4.4-2. It requires that the minimum initial sample for inspecting four (4) steam generators L be three (3) percent of the total tubes installed in each generator. In addition, Technical

- Specification 3/4.4.5.2.e requires the following: "For Unit 1, implementation of the steam generator tube / tube support plate repair criteria requires a 100-percent bobbin coil inspection for hot-leg and cold-leg tube support plate with known outside diameter stress corrosion cracking (ODSCC) indications."

' The "1997 Outage Plan for the In-senice Inspection of Steam Generator Tubing at the South

-lexas Project Electric Generating Station Unit 1" required one hundred (100) percent of the tubes be examined by the bobbin coil method of ET, full length, and MRPC ET at TSH in the initial sample. -It also required approximately twenty-two (22) percent of the tubes to be' examined by

The examination resuhs for the 100 percent tube inspection by babbin coil ET and at the TSH -

with MRPC ET should be categorized as Category C-2. Some, but not all, of the examination j resuhs for the first, second, or third sample of the MRPC ET at TSC shoukt be categorized as Category C-2.: Should all of the initial and subsequent examination resuhs have been conservatively categorized as C-2, the consequence, according to Table 4.4-2 of the Technical Specification, is the addition to the initial sample of eighteen (18) percent additional tubes to be inspected. Full length bobbin coil ET and MRPC ET at TSH were performed on all in-senice -

tubes. MRPC ET at TSC was performed on more than twenty-two (22) percent of the in-service ,

tubes of each steam generator. Therefore, the Technical Specification requirements for additional ,

examinations required in response to examination results were met.

The tubes which were amoved from service by plugging prior to this ISI and as a resuk of this - l ISI are identified on tubesheet maps, one for each steam generator, in Appendix E. The symbol X on each map represents those tubes plugged during this outage and " box symbol" (" Existing Plugs") are those locations removed fmm senice prior to 1RE07.
