Gentlemen:
This refers to the inspection conducted by Messrs. C. E. Johnson, A.- Singh, and
R. V. Azua of this office during the period May 16-17 and 23-27,1988, of
activities authorized'by NRC Operating License DPR-45 for the Cooper Nuclear
Station (CNS), and to the discussion of our findings with Mr. G. R. Horn, and
other members of your staff at the conclusion of the inspection.
Areas examined during the inspection included the containment integrated leak
rate test and the operability evaluation of essential piping systems.
Within
these areas, the inspection consisted of selective examination of procedures
and representative records, interviews with personnel, and observations by the
NRC inspectors.
The inspection findings are documented in the enclosed
inspection report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements.
An individual, without direction and of his own
I
volition, altered the position of several valves that had been visibly tagged
for the containment integrated leak rate test, thus violating the procedure for
this test.
The technician took this action thinking-that he was correcting an
improper valve alignment.
His actions had the potential to invalidate the test
if they had not been identified in time.
This is considered to be a serious
breach of discipline.
It puts into question the effectiveness of CNS's
training program when an individual could jeopardize a test due to his
seeming lack of understanding of the purpose of equipment and valve tagging.
You are required to respond to this violation, in writing, in accordance with
the provisions of Section 2.201 of the NRC's "Rules of Practice," Part 2,
Title 10, Code of Federal Regulations.
Your response should be based on the
specifics contained in the Notice of Violation enclosed with this letter.
In
your response, please address management's actions to assure a discipline
exists among plant workers that would prevent the unauthorized manipulation of
tagged equipment.
The response directed by this letter and the acco...panying Notice is not subject
to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, PL 96-511.
