P APPENDIX A NOTICE OF VIOLATION Nebraska Public Power District Docket:
50-298 Cooper Nuclear Station Operating License:
OPR-46 During an NRC inspection conducted during the period May 16-17 and 23-27, 1988, a violation of NRC requirements was identified.
The violation involved failure to follow a containment integrated leak rate test (CILRT) procedure.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Fart 2, Appendix C (1988), the violation is listed below:
Failure to Follow CILRT Procedur_e Criterion V of Appendix B to 10 CFR 50 requires, in part, that activities affecting quality be prescribed by documented procedures and accomplished in accordance with these procedures.
Cooper Nuclear Station Surveillance Procedure 6.3.1.3, "Primary CILRT,"
Revision 10, dated April 4, 1988, specifies in Sections 8.4.1 through 8.4.4 that pre-test alignment of valves shall be performed as stated in Attachments 7 and 8, and that the valve positions should be verified to avoid any ertars.
Section 5.5 specifies that equipment und valve alignments necessary for proper testing requirements shall be indicated by green testing tags, and that the position indicated on the tags should not be altered without contacting the shift supervisor and the test director.
Contrary to the above, on May 24, 1988, it was observed by the NRC inspectors that valve No. PC-80 was not in the proper position and that the cap for valve No. PC-74 was not removed as indicated on Attachment 8, and on the green integrated leak rate test (ILRT) tags.
On May 25, 1988, an individual altered the position of some valves and caps, identified in, without notifying either the shift supervisor or the test director.
This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
