Gentlemen:

This refer; to the routine safety inspection conducted by Mr. H. A. Walker and

others of this office on April 18-22, May 2-6, 11, 31 and June 1,1988, of

activities at the Quad Cities Nuclear Power Plant, Ur.its 1 and 2, authorized

by Operating Licenses No. OPR-29 and No. DPR-30, and to the discussion of our

findings with Mr. R. Bax and others of your staff at the conclusion of the

inspection.

The purpose of the inspection was to assess the accomplishment

and effectiveness of maintenance activities.

The enclosed copy of our inspection report identifies areas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and

interviews with personnel.

This inspection and the recent NRC inspection of the High Pressure Core

Injection and Reactor Core Isolation Cooling Systems at the Quad Cities

facility raised substantial concerns about the perforniance of maintenance,

modifications, and the evaluation of problems for root cause.

These concerns

are illustrated by the following:

Installation of a ground in the 1/2 Emergency D'esel Generator (EDG)

auto-start circuitry and subsequent failure to correct this ground

despite an indication that a problem existed during post-modification

testing.

Routinely operating the plant with numerous grounds in the DC electrical

system. Operations in this manner can result in masking faulted

conditions which can prevent safety-related components from performing

their intended safety 'JnCtions,

Corrective actions after component failures to similar components or

the same component in the adjacent unit were inadequate.

This was the

case with the 4KV EDG breaker trip latch mechanism failure that occurred

in October 1987 and with repeated 480 volt auxiliary electrical contact

failures that had occurred since 1980.

These deficiencies indicate the need for management attention to the

performance of maintenance and modifications.

During this inspection, it

was noted that management had already initiated action plans to improve these

areas.

However, based on the significance of deficiencies identified during

this inspection, the NRC will closely follow your progress in.these areas.

We are releasing the report at this time for your information.

The apparent

violations, described in Paragraphs 3.3.1.1.2 and 3.3.1.1.3 of this report are

being reviewed for potential enforcement action.

An enforcement conference

will be held on June 24, 1988 in the Region III Office with you and other

members of your staff.

You will be notified by separate correspondence of

our decision regarding enforcement action.

No written response is required

until you are notified of the proposed enforcement action.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget

as requiret by the Paperwork Reduction Act of 1980, PL 96-511.

We will gladly discuss any questions you have concer

this inspection.
