Nuclear Generating Station Units 1 and 2, authorized by NRC Operating Licenses

No violations of NRC requirements were identified during the course of this

inspection. However, Exercise Weaknesses were identified, as sumarized in

the Appendix to this letter. As required by 10 CFR Part 50, Appendix E(IV.F),
any identified weaknesses must be corrected. Accordingly, you are requested

actions you have taken or plan to take, showing the estimated date of

One weakness was the inadequate overall response to the Transportation
,

Accident that involved personnei injuries with and without contamination
complications. As discur. sed and agreed to in the October 14 teleconference
between our respective staffs, another demonstration of response capabili'.ies
to such a situation, rather than our observation of remedial classroom training
activities or training records review, is needed.

APPENDIX A
EXERCISE WEAKNESSES
1.
Neither Control Room nor Technical Support Center staff adequately
informed State officials of the Transportation Accident situation.
They failed to utilize the "IESDA Hazardous Materials Radiological
Questionnaire" that had been developed for such situations and
incorporated into the Emergency Plan.

Onshift personnel failed to recognize, in a timely manner, that
degraded plant conditions warranted a Site Area Emergency declaration.
