Reactor trip occurred. (reason unspecified)

HPCI was unavailable due to maintonance.

ROIC initiated on low-low reactor water level (-38 inches).

The RCIC turbine tripped on overspeed.

ROIC was reset and manually set into operation.

The reactor water level had reached -40 inches.

Corrective action:

An investigation revealed a crack in the amphenal connector to the tachometer pickup causing a loose connection.

This connector was replaced and the turbine tested with satisfactory results.

1. The RCIC provides reactor core cooling and water level control during a loss of feedwater flow.
2. The tachometer monitors the turbine speed for control purposes.
