ovember 20, 1979 ROIC Turbine Trips with HPCI Unavailable at Brunswick 2 re sequence was:

tor trip occurred. (reason unspecified) was unavailable due to maintonance.

initiated on low-low reactor water level (-38 inches).

RCIC turbine tripped on overspeed.

was reset and manually set into operation. The reactor water level had hed -40 inches.

e action:

investigation revealed a crack in the amphenal connector to the tachometer ausing a loose connection. This connector was replaced and the turbine ith satisfactory results.

urpose of failed system or component:

RCIC provides reactor core cooling and water level control during a loss of water flow.

tachometer monitors the turbine speed for control purposes.

ility of system per WASH 1400:

  • RCIC: 8 x 10-2/D ility of component per WASH 1400:*-

vailabilities are in units of per demand D-1 . Failure rates are in per hour HR-1

Damage No No - ADS/CS/LPCI available ahould both HPCI and RCIC fail No No I_______________________________
