b APPENDIX A MTICEOFVIOLATION Louisiana Power & Light Company Docket: 50-382 Waterford Steam Electric Station, Unit 3 Operating License: NPF-38 During an NRC inspection conducted March 16 through April 30, 1988, three violations of NRC requirements were identified. The first violation involved failure to provide and implement adequate work instructions for a safety-related component. The second violation involved failure to provide special reports required by Technical Specifications. The third violation' involved failure to provide an adeouate operating procedure. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violations are listed below:

A. Inadequate Maintenance Work Instructions Technical Specification 6.8.1.a requires written procedures to be established, implemented, and maintained covering the applicable

procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Regulatory Guide 1.33 requires procedures appropriate to the circumstances for performing maintenance on safety-related equipment.

Contrary to the above, on April 18, 1988, the maintenance procedure / work instruction authorizing disassembly of Main Steam Isolation Valve MS-124B was not appropriate to the circumstances in that maintenance personnel were applying hydraulic jacks to unseat the valve and the work instruction did not address this activity.

B. Failure to Report Nonvalid Diesel Generator Failures Technical Specification 4.8.1.1.3 requires that all diesel generator failures, valid or nonvalid, shall be reported in a Special Report to the Comission pursuant to Specification 6.9.2 within 30 days.

Contrary to the above, the diesel generators exhibited nonvalid failures on August 1, 1985; January 30, 1986; March 7, 1986; May 8, 1987; June 22, i 1987; June 23, 1987; August 15, 1987; and six more between October 19, i

> 1987, and March 8, 1988. The licensee failed to report these failures  ;

until a sumary report was issued on March 18, 1988, covering all of the '

nonvalid failures.

C. Inadequate Operating Procedure Technical Specification 6.8.1.a requires written procedures to be  ;

established, implemented, and maintained for atmosphere clea' up systems as recommended in Appendix A of Regulatory Guide 1.33, Revision 2,.

Contrary to the above, on April 29, 1988, the licensee failed to establish an adequate procedure to control the operation of the fuel handling ventilation system, in.that the procedure valve lineup failed to identify 14 instrument root valves and specified the incorrect position for several damoers.

Pursuant to the provisions of 10 CFR 2.201, Louisiana-Power & Light Company is ,

hereby required to submit to this office within 30 days of the date of the

letter transmitting this Notice, a written statement or explanation in reply, including for the "A" and "C" violations: (1) the reason for the viclation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time. No reply is required for the "B" violation. Louisiana Power & Light Company has  ;

already taken the appropriate corrective action as to cause. These actions are described in NRC Inspection Report 50-382/87-25 dated. December 30, 1987, and in the attached NRC Inspection Report 50-382/88-08.
