I'nspection No. 50-213/86-02
A routine, unannounced inspection was conducted February 10-14, 1986 of the-
Haddam Neck Plant refueling outage health physics program. Areas reviewed
included qualification and training of licensee and contractor personnel,
personnel exposure control, contamination control, ALARA, and dosimetry
records.
Review of the licensee Radiological Incident Reporting system indicated that
although incidents are being identified, subsequent corrective action in one
occurrence was inadequate.
In this occurrence a compacting operation was
conducted in a manner different from the usual procedure and in a way that was
not. subject to safety evaluation, contrary to regulatory requirements. As a
result several personnel were subject to internal contamination. Details are
provided in enclosed Appendix A.

by' following the instructions included in Appendix A.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter and your reply
will be placed in the Public Document Room.
