Gentlemen:

This refers to the safety inspection conducted by Messrs. C. F. Gill,

W. B. Grant, and D. E. Miller of this office on January 7-10, 23-25, 29, and

30, and February 28, 1986, of activities at Palisades Nuclear Generating Plant

authorized by NRC Provisional Operating License No. DPR-20 and to the discussions

of our findings with Mr. J. Lewis and others at the conclusion of the inspection

on January 10 and 30, 1986.

The enclosed copy of our inspection report identifies areas examined during

the inspection, including review of the excessive radiation dose recorded by a

worker's dosimetry on January 21, 1986. Within these areas, the inspection

consisted of a selective examination of procedures and representative records, .

observations, and interviews with personnel.

During this inspection, certain of your activities appeared to be in

violation of NRC requirements, as specified in the enclosed Notice. A

written response is required. Additional violations of NRC requirements,

which you identified during your investigation of the January 21, 1986,

incident are contained in Section 17 of the enclosed inspection report. Due

to your prompt and comprehensive evaluation and correction of this incident,

we have exercised our discretion as allowed under the NRC enforcement policy,

and have not included these items in the enclosed Notice of Violation.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
