Subject: Inspection No. 50-293/88-17

This refers to the maintenance assessment team inspection led by

Mr. D. Haverkamp of this office on April 25 to May 5,1988, at the Pilgrim

Nuclear Power Station (PNPS), Plymouth, Massachusetts and at the Braintree,

Massachusetts engineering office. The inspection addressed activities

authorized by NRC License No. DPR-35. At the conclusion of the inspection,

an exit interview was held with you and members of your staff to discuss

the scope and the findings of the inspection. The inspection findings

were also discussed during a telephone conversation on May 25, 1988 between

Mr. R. Anderson of your staff and Mr. Haverkamp.

The purpost of this inspection was to perform an in-depth assessment of main-

tenance activities at PNPS, including examination of identified maintenance

problems, correction of those problems, and maintenance section interfaces

with engineering, operations, quality assurance and health physics personnel.

Particular emphasis was placed on maintenance activities associated with the

high pressure coolant injection system and reactor core isolation cooling

system that were completed during the current outage, as well'as ongoing

maintenance activities.

Areas examined during this inspection are described in the NRC Region I

Inspection Report which is enclosed with this letter. Within these areas,

the inspection consisted of interviews with personnel, observations and

examinations of plant equipment and selective examinations of procedures,

representative records and documents by the inspectors. No violations of

regulatory requirements were identified during the inspection.

However, significant deficiencies were identified during the inspection that

warrant your attention and should be addressed prior to PNPS restart. The

deficiencies are in the areas of the maintenance program, program performance

and staffing, and are identified in section 1.2 of the enclosed report. We

request that you responJ in writing within thirty days of receipt of this

letter identifying actions taken or planned to address these deficiencies.
