APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Generating Station License No. CPPR-120 As a result of the inspection conducted on January 13 - February 9,1986, and in accordance with NRC Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:

10 CFR 50 Appendix B Criterion XI, Test Control requires a testing program to assure all testing, including preoperational tests, required to demonstrate that structures, systems, and components will perform satisfactorily is identified and performed in accordance with written test procedures which incorporate the requirements and acceptance limits of design documents.

Contrary to the above as of January 7,1986, adequate testing including preoperational testing required to demonstrate satisfactory performance of the Core Spray System had not been identified or performed. Portions of the Core Spray initiation logic, diesel power monitor, had not been in-stalled. Component level testing to verify proper installation had not identified the condition. Completed and approved system preoperational test PTP-BE-1, Revision 0, Core Spray System, did not include steps to fully test the core spray initiation logic and therefore failed to fully demonstrate that the Core Spray System would perform satisfactorily.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
