This refers to the inspection conducted by Messrs R. E. Farrell, P. W. Michaud,
and R. P. Mullikin of this office and K. L. Heitner of the Office of Nuclear

Reactor Regulation during the period March 1-31, 1988, of activities authorized
!
by NRC Operating License DPR-34 for Fort St. Vrain Nuclear Generating Station,
and to the discussion of our findings with you and other members of your staff
at the conclusion of the inspection.
Areas examined during the inspection . included followup of licensee action on

previously identified findings, operational safety verification, review of the
10 CFR Part 21 reports, licensee action on licensee event reports, engineered
safety features (ESF) walkdown, region peaking factor surveillance,10 CFR 50.59 safety evaluations, monthly surveillance observation, monthly maintenance
observation, radiological protection, and monthly security observation. Within
these areas, the inspection consisted of selective examination of procedures
and representative records, interviews with personnel, and observations by the
inspectors. The inspection findings are documented in the enclosed inspection
report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements. Consequently, you are required to respond to
these violations, in writing, in accordance with the provisions of
Section 2.201 of the NRC's "Rules of Practice," Part 2, Title 10 Code of
Federal Regulations.
Your response should be based on the specifics contained

in the Notice of Violation enclosed with this letter.
This inspection identified three violations. Two of these violations involve
failure to follow procedures and the other involves inadequate procedures.
Recent previous inspections have identified similar violations. Your response
to these violations should address the recurring nature of these violations and
the programatic actions being taken to prevent future violations of this nature
in all activities related to Ft. St. Vrain.
In addition, you should consider
a complete review of your Change Notice development process as a result of
weaknesses identified in your program.
Incomplete references and incomplete
documentation were discovered by the NRC inspectors in the Change Notice
process as a result of the review of the 10 CFR 50.59 safety evaluations.

to the clearance procedures of the Office of Management and Budget as required
