APPENDIX A NOTICE OF VIOLATION Nebraska Public Power District Docket:
50-298 Cooper Nuclear Station Operating License:
DPR-46 During an NRC inspection conducted on April 11-15, 1988, a violation of NRC requirements was identified.
The violation involved failure to follow an emergency preparedness procedure.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:
Failure to Follow Procedures Technical Specification 6.3, Procedures and Programs, states that written procedures and instructions, including applicable check-off lists, shall be established, implemented, and maintained for implementing procedures of the Emergency Plan.
Emergency Plan Implementing Procedure 5.7.21 states that "This procedure provides a means of ensuring readiness and availability of equipment required for the inaediate action steps of all four emergency classification action levels."
Contrary to the above, the emergency telephone directory located in the Emergency Operations Facility and Operational Support Centers contained outdated information.
Additionally, one emergency kit located at the Nemaha County Hospital was missing masking tape and a cardboard storage box.
This is a Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, Nebraska Public Power District is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid 'urther violations, and (4) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
