n APPENGIX A NOTICE OF VIOLATION
' Gulf States Utilities Docket No.:
50-458 River Bend Station License No.: NPF-47 During an NRC inspection conducted on January 13-17, 1986, a violation of NRC requirements was identified.
The violation involved failure to properly implement the temporary alteration procedure.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:
Technical Specifications, paragraph 6.8.1 requires that procedures'be established and implemented for safety-related maintenance and modification work.
Administrative Procedures ADM-0031, " Temporary Alterations," Revision 2, paragraph 5.2.6 states, in part, that "... the request shall be executed by the worker assigned to the job, and be independently verified...
as necessary...."
Contrary to the above, temporary alterations 85-RHS-28 and 85-RHS-29 were executed without the required signatures upon installation and the required independent verification.
This is a Severity Level IV violation (Supplement I.D).
Pursuant to the provisions of 10 CFR 2.201, Gulf States Utilities is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: -(1) the r'eason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the correc-tive steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
