Gentlemen:

Enclosed for your review, prior to our scheduled meeting of February 18, 1986

is the SALP 5 Board Report for the LaSalle County Nuclear Power Station, covering

the period May 1, 1984 through September 30, 1985.

In accordance with NRC policy, I have reviewed the SALP Board assessment and

concur with the ratings.

These ratings clearly show a decline in operational

-performance at the LaSalle station over the assessment period, which is a matter

of significant concern to me.

In fact, I would have to rate the LaSalle

regulatory performance as one of the lowest in Region III.

I would like to

highlight the following items:

1.

LaSalle received a Category 3 rating in four f mctional areas including

all three functional areas directly involved with operations (plant

operations, maintenance / modifications, and surveillance / inservice

testing).

Furthermore, the performance trend within the assessment

period for the maintenance and surveillence areas declined, while the

trend for the operations area was mixed.

A Category 3 rating was also

given for the area of quality programs and administrative controls.

Performance in the area of radiological controls, although it remained

a Category 2 rating, also declined.

LaSalle received two Category 1 ratings (in fire protection / housekeeping

and in startup testing) and in both cases this was an improvement

over the Category 2 rating achieved in the previous assessment

period.

Throughout the SALP period, we have had several meetings to discuss. specific

problem areas that have arisen at the LaSalle plant.

To aid in our understanding

of operations at LaSalle, we conducted a special Task Force review of LaSalle

operations and, as a result, issued a 10CFR 50.54(f) letter on November 22,

1985 describing our concerns.

In that letter we requested that you evaluate

i

five major areas of concern:

1) adequacy of management structures and practices;

2) adequacy of the maintenance and modification programs; 3) adequacy of

control of work activities; 4) adequacy of the implementation of the Regulatory

Performance Improvement Plan; and, 5) adequacy of the resources committed to

L

the LaSalle County Station.

We have received your response and have it under

review. We believe that corrective actions to address these broad concerns

will also substantially address the findings of this SALP report.

Consequently,

we are not asking you to specifically respond to this letter and report.
