NOTICE OF VIOLATICN Detroit Edison Company Docket No. 50-341 As a result of the inspection conducted on March 7-25, 1988, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1987), the following violations were identified:
10 CFR 50, Appendix B, Criterion V as implemented by the Fermi 2, QA Program for Plant Operations, Section 17.2.5, "Instructions, Procedures, and Drawings,"
requires that activities affecting quality be prescribed by documented instructions and procedures of a type appropriate to the circumstances and that work be accomplished in accordance with the procedures. Section 17.2.5 further requires procedures to include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished.
Contrary to the above:

No acceptance criteria or machining dimensions and tolerances were provided in the main steam isolation valve (MSIV) procedure (P0M 35.000.032(SQ), Revision 8) or work request (0238012788) for MSly B2103F022B, yet the main seat of the poppet for the MSIV was inspected and machined based solely on verbal direction from the MSIV vendor representative (341/88007-3A).
b.
A fuel injector pump for the diesel driven emergency generator EDG-13 was removed, inspected and re-installed in accordance with verbal instructions from the vendor representative instead of by following approved procedure POM 35.000.052(SR), "Emergency Diesel Generator -
Engine General Maintenance," Revision 6 (341/88007-38).
Hydrostatic pressure testing of the jacket water system of EDG-13 was c.
not accomplished in accordance with approved procedure POM 34.000.14(SQ),
"Emergency Diesel Generator - Inspection," Revision 8 (341/88007-3C).
d.
Component replacements made on the control panel for EDG-13 were not classified and controlled as a modification in accordance with the requirements of NOIP 11.000.004(50), "Design Change Process,"
Revision 2, Enclosure A (341/88007-3D).
This is a Severity Level IV violation (Supplement 1).
