Dear Mr. Caylor:
l As requested in your letter dated July 1,1998, Quality Assurance Program Approval No. 0420 l is hereby terminated.
l Please note that records required by 10 CFR S 71.91 and 71.135 and not transferred to SteriGenics Intemational are to be retained for a period of three years as follows:
l Records of each shipment of licensed material shall be maintained for three years after that shipment (10 CFR S 71.91(a)]. I Records providing evidence of packaging quality shall be maintained for three years after the life of the packaging (10 CFR S 71.91(c)]. l Records describing activities affecting packaging quality shall be maintained for three l years after this Quality Assurance Program Approvalis terminated (10 CFR S 71.135).
This letter also serves as a reminder that your authority to use Certificate of Compliance No. 5939 under the general license provisions of 10 CFR e 71.12 is also terminated.
