l It has come to our attention that the corporate name as shown on Quality Assurance (QA) l Program Approval for Radioactive Material Packages No. 0420, Revision No. 7, dated l
February 21,1996, may no longer be accurate. This QA Approval shows Process Technology, North Jersey, at the same address as SteriGenics Intemational, as the authorized corporation.
l If the name of the corporation holding this Approval has changed, you should request an amendment to your QA Program indicating the change of corporate name and any other changes that may have been made since your QA Program was approved. A fee of $640 should be included with your request for this amendment.
