ADMISSION OR DENIAL OF THE ALLEGED VIOLATION South Carolina Electric & Gas Company (SCE&G) is in agreement with the alleged violation.
II.
REASON FOR THE VIOLATION On June 7, 1987 during performance of Surveillance Test Procedure (STP) 210.002, "Isothermal Temperature Coefficient Measurement,"
following startup after the third refueling outage, the Licensee incorrectly calculated the moderator temperature coefficient (MTC) for the all rods out configuration.
Specifically, the Licensee incorrectly scaled the temperature data used in the calculation which resulted in a non-conservative estimate for MTC. However, this value for MTC was within Technical Specification limits.
The cause of this violation has been attributed to personnel error.
The procedure used STP-210.002, did not require an independent verification of the calculated core physics coefficients, which were in error due to the misapplication of the temperature scaling factor.
III.
CORRECTIVE STEPS TAKEN AND RESULTS ACHIEVE 0 When the crror in the MTC calculation was first identified by the NRC Region II i,1pector, the Licensee immediately recalculated the MTC and verifiec it to be within Technical Specification limits.
Temperature and reactivity data from the strip chart recorders used during the startup (independent of the x-y plotter) were reduced and a second verification of the MTC was performed.
IV.
CORRECTIVE ACTION TAKEN TO AVOID FURTHER VIOLATION The Licensee will change all safety-related Reactor Engineering and 200 series (Reactor Engineering) Surveillance Test Procedures to require an independent verification of all procedure results.
In addition, STP-210.002 will be changed to explicitly identify the equations used in calculating core physics coefficients and to require two independent methods of determining MTC: one from the x-y plotter and the other from the strip chart recorders.
V.
DATE OF FULL COMPLIANCE SCE&G will be in compliance with respect to the corrective actions as stated no later than November 1, 1988.
