(NRC INSPECTION rep 0RT NOS. 50-327/88-20 AND 50-328/88-20)
This refers to the special Nuclear Regulatory Commission (NRC) heatup
shift coverage inspection conducted by the NRC's Sequoyah Restart Task
i
Force on February 27, 1988 thru March 18, 1988.
The inspection included
a review of the activities associated with the heatup of Sequoyah
Unit 2.
At the conclusion of the-inspection, the findings were
discussed with those members of your staff identified in the enclosed
inspection report.
Areas examined during the inspection are identified in the report.
These areas included extended control room observation and operational
safety verification.
Within these areas, the inspection consisted of
selective examinations of procedures and representative records,
interviews with personnel, and observation of activities in progress.
The ir.spection findings indicate that certain activities appeared to
violate NRC requirements.
The identified violations involved: (1)
failure to develop or implement procedures (3 examples), (2) failure to
comply with cold leg accumulator Technical Specification requirements
(missed surveillance test), (3) failure to comply with TS 3.0.3
involving the loss of safety functions, and (4) failure to ensure timely
notification of the NRC of a loss of safety functions as required by
10 CFR 50.72.b.2.111 and AI-18, "plant Reporting Requirements".
The
first and second violations, which are described in paragraphs 10 and 11
of the enclosed report, will be addressed under separate correspondence.
Additionally, the third and fourth violations, which are described in
paragraph 11 of the enclosed inspection report, are under consideration
for escalated enforcement action.
Accordingly, a Notice of Violation
addressing each of the violations is not being issued at this time, and
therefore no response to these violations is required.
However, please
be advised that the characterization of the violations described in
paragraphs 10 and 11 of the enclosed inspection report may change as a
result of further NRC review.
You will be advised by separate
correspondence of the results of our deliberations on this matter.
